Legal issues, mostly. Players from quite some US states, and all the canadians were not allowed to play. Something about games, winning money, gambling, etc. That obviously disqulified or gimped many teams, which in turn didn´t play on the quali server.
While that was problematic in the EU too, the problem seems to have been much worse in the US. Which lead to the fact, that there hardly was any competition at all.
