The way it works is just scanning your raid for targets and then firing Ace2 events when the target status changes. Currently BanzaiLib supports 4 events: Banzai_UnitGainedAggro, Banzai_PlayerGainedAggro, Banzai_UnitLostAggro and Banzai_PlayerLostAggro, and has 2 API methods: GetUnitAggroByUnitId and GetUnitAggroByUnitName.
From the www.wowinterface.com description of BanzaiAlert. As you know you can see party targets with some unit frames (I believe PitBull does this). BanzaiAlert takes this a step further by checking whenever any target changes of any of the targets of your raid. If the new target is you and the targetting unit is an enemy, it triggers BanzaiAlert. Therefore if ANY of your raid members is targetting ANY of the mobs that you gain aggro on during fighting, the alert will show up.
