It seems that we had a ridiculous amount of C'thun wipes because of people jumping around. Someone actually managed to cause two in a row, without a proper pull between them. It was at that point that most people started hugging the back wall and making sure everyone else was well away from them (which was especially fun for buffing).
We used to tell people who had never been there before the room of blobs before Patchwerk had to be body pulled. They'd pretty much always die without even asking why them, or why they had to be body pulled. I think only one or two new people out of about 20 total ever stopped and wondered why.
Frogger was good - totems cause them to explode so most of out shamans would put down totems and then remove them randomly, causing a number of deaths. Stealthed rogues caused a lot too.
One of our shamans at Thaddius, using the bigwigs Arrows mod, managed to screw up which side. After our raid leader shouted at him he realised he had his camera facing backwards ....
Much more recently, on Mother Shahraz, one of the hunters misdirected it to our MT, as always. About 5 seconds later our MT intercepted Shahraz only to get insta-killed (unsurprisingly). Turns out his sunder button is the same as intercept (def stance/zerk stance) and he forgot to switch stance after using Zerk rage. That was an especially quick reset.
