We had a pally who not once, but twice BOP'd a tank on Al'ar accidentally.
The MC "window to Rag" is a classic I think everyone has pulled on new people.
In my old guild we had a rule that people had to be at the zone in at raid time or lose priority on a spot so we would summon the slackers into the lava in BRM for bwl nights.
I have a couple times done Kael'thas attempts with no weapon equiped because I forgot to re-equip after a wipe when the staff despawns.
