After experiencing this and some testing, I found if Grace of the Naaru (proc from Libram of Mending) is active, I get no MP5 from [Pendant of the Violet Eye] click. If no Grace of the Naaru, Pendant works fine. As I keep Grace up constantly, I finally swapped out my Pendant 
This has been happening since at least 2.4 - I didn't use LibMending before that. |
For what it's worth, we had a holy paladin complaining about this exact issue last night. He logged out in his PvP gear so I can't definitely see what Libram he was using.
