I just remembered one more from a while back. When we were first learning Illhoof, none of us knew that the pack of elite imps just inside the doorway wasn't linked to the boss. So every first attempt we made on him ended in a wipe because we tried to AoE down the elite imps whilst engaging the boss. We even tried a strategy where we'd basically throw ourselves to our deaths to try to clear out the imp pack before our first "real" attempt of the evening. We only found out that they weren't linked later, when we pulled in a new raider to fill in the gap left by our previous raid leader. He facepulled that pack, sending us scampering in all directions, and we came to the realization that the boss hadn't pulled with them.
