I did pretty much the same thing myself, but at least it was in beta. Rolled an undead warlock the night we got in, with 2 friends who rolled orc and troll - so we weren't playing together to start. I got to level 10 before finding out from them that you could upgrade your spells at a trainer - I did wonder why my progress was starting to slow down so much.

One of our DPS warriors got mind controlled shortly after getting Halberd of Smiting the first time we did ZG, this while the MC gave a +dmg debuff...activates recklessness, intercepts into a group of people and whirlwinds...down went 5 people in one hit.
Best one however was we were doing a PUG AQ20, just killed Kurinaxx so the Master Looter links the drops then dissapears! o.O Next we hear a loud "F******CK!" on the teamspeak we'd been using, he'd joined the queue for AV and had forgotten to turn off his auto accept invites mod. We'd had no lock in the group but somehow managed to get him back to silithus before the corpse despawned.
Also had various other instances of people randomly vanishing from instances because of these mods.

We were learning Maulgar with 2 hunters present. Hunters tank Kriggler, but we still had to MD pull Maulgar to the MT. So I popped MD, distracting shot Maulgar, then pelted Kriggler a couple times. Krigger then ran over and lightning bolt/arcane shocked the MT, who died shortly after. I forgot to click off MD. Only people in hunter chat ever knew what exactly happened.
In Naxx during Thaddius I remember being grouped with certain people every attempt to pack-taxi them for the phase 2 transition jump because they could never make the jump, ever (most notably, all of our shams).

I used to play with my wireless keyboard on my lap. One time during Vael my chair tipped back unexpectantly and sent my keyboard flying into the air, with my scrabbling to grab it before it smashed. Phew I caught it! Looked at the screen to see myself Arcane Missiling Vael whilst we're buffing. Whoops.

I'm expecting my raid-leader to macro something about melee and Krosh Firehand any raid now, considering he's been using /rw to shout at the survivors for the last three weeks (which is when we got our first kill). It's getting to the point where I can predict our add-killing progress - when there's a big spike of damage to our melee, we've probably reached the mage on the kill order.

Back in MC one of our priests fell asleep in the Garr room when we were about to pull Geddon.
Naturally, I told our MT to tank Geddon on top of the poor priest and made sure no one healed the priest by announcing "DPS go as soon as the priest is dead". Good times.

I managed this once, but not by being drunk. I was at a LAN and we were doing Naxxramas with the guild (Praise the lord most big LAN parties have broadband connections nowadays) and I was RL/ML. So we go to Noth and we start fighting him and due to being so tired, I play like a idiot and pull aggro on Noth and die. I mention in raid chat I'll be afk for a moment, so everyone knows. They keep fighting.
For some reason, I went to lay down on my "bed" and I just fell asleep. I hadn't slept for 2 days on that LAN so I was pretty knackered.
Unfortunately, I didn't wake up until one of the guild members that was also at that LAN came to wake me up. He forgot I was at that party too, so it took him about as long to figure out he could come wake me up... unfortunately, too long - the corpse had already despawned.
Luckily, the GM we contacted about it thought it was pretty funny and he offered us the loot from him. It was good loot for once too! Hatchet of Sundered Bone + Band of the Inevitable and a Splinter!
The irony? I got both the splinter and the band.

The last time somebody in my guild used the term "snowballing X" when describing that, many dirty jokes were exchanged afterwards.

Oh man... Where to start... I have done so many stupid things in raids, I don't even know where to begin. The one I get the most shit for is falling off the bridge on top of Golemagg in MC, needless to say everyone was stoked. I've also fallen off Hakkar's platform while pulling Sons, backed off of Hydross' platform mid fight and had to swim back. I fall a lot... Hmm.
One of my favorite stupid environmental bugs was getting feared into walls on Nightbane... One time I got feared off of the terrace and fell down to an area I had never seen before but was able to attack through the ceiling for a little while. In an attempt to get back up to the fight I ended up in one of the spider rooms.
Stupid things I've had the privilage of seeing others do: Basically every MC, someone blew the group up on Geddon. One genius went ninja afk before that fight and put himself on follow... We had to get him stuck in a wall. Shamans totem aggroing channelers on Mag while we are buffing is good for a laugh. Someone aggroing the audience after an Opera kill. People's complete inability to not move during Flame Wreath on Aran, then trying to blame it on a hunter pet when you can just scroll through your combat log and read their name off it. The amount of deaths to fish and elevators in SSC still won't cease to amaze me.
I suppose this is all pretty standard fare.

My first time doing Ragnaros, on my old warrior, I was still a huge noob. the concept of "move out of melee range" just didn't quite register. i died, i think every pull that night.
Oh, and I don't remember this, but my old warrior class leader told me recently that when I first joined, I was using a +healing mace, supposedly because I thought it added to the healing I was receiving. I cannot confirm this, but he is certain that it was me or another warrior in the guild. Either way, thats pretty bad.

-Razergore: "Why did you snowball me are you $#@*^&@ stupid!?" - Shaman who kites
-C'Thun: "I thought the beam was going the other way..."
-Viscidus: [Raid leader] "Wipe it wipe it, run out."... death screams from below as people fall from the bridge and crater
-Patchwerk: (stupid melee X) dies in the acid before engaging.
-Thaddius: "I fell again."
-Twin Emps: "Uhhh there's bugs on me"
-Magtheridon: "I clicked, I clicked WTF!"
-Gruul: (Ground stomp) [5-6 people simultaneously on vent] "WOAH CRAP".
-Gruul: (Ground stomp) and all warriors intercept back and stack on top of each other
-Vashj: Throwing the ball at a shaman who is in the oncoming path of a strider.
-Teron: (Player is ghosted) "I just bound the keys to my numpad, why isn't it working!?" -"Try numlock?"

Jump by Thaddius was the worst. I was a victim of this multiple times.
I think that and the Heigan "safe spot" with the fix in one of the patches, which gibbed like 15 casters, hunters and healers was pretty cool.

I'm sure everyone has had at least one bad pull at Hydross, and there are a number of ways to make it easier. I think there's a thread about it.
Well our tanks have typically gone in with swiftness pots, but one week we decided to try an invisibility potion instead.
The tank says he's going in, pops his potion, and runs over to hydross. Everything is great. He's in position. A few seconds later Hydross comes barelling over the the line and smacks a paladin (our raid leader, actually).
Apparently our tank was overjoyed with the success of his invisibility potion and was standing behind hydross dancing. Our raid leader was just getting ready to heal him.

This is probably my crowning glory.
YouTube - C'thun OMG laser
Little back story, this was our first engage on c'thun way before there were any spoiler sites or boss strats were openly discussed. So the GL is counting down, he gets to 2 and i start saying "WAIT WAIT!" which was an ongoing joke in that guild, some one (usually me) would always interrupt the engage countdown. GL says something to the extent of "FFS, what now" lots of laughing results, I'm the guy that suggests testing agro range. There is a 10 second silence, aside from random giggling, where the GL answers my question by restarting the countdown. Well, one of our mages thought testing the agro range was a good idea. The result was probably my favorite raiding incident ever.

I'm guilty of leaving Water Walking on for Lurker.
And we had a pally BoP the Tidewalker MT, and then the next night DI the tank on Illhoof.

I have successfully BoP'd the tank on fights such as Tidewalker and Illidan. The Tidewalker BoP we lived through, but the Illidan one we died on. Our Warlock tank caught aggro and asked for a BoP with like 15% left and as that happened our tank actually picked up the aggro. However, that being said, my BoP landed on the tank we wiped in dramatic fashion.

One long night in MC, after our MT yelling at people for 10 minutes at the start to make sure they had enough pots, regents, ect., he has to Zone out to get more fire prot pots from the bank. As the raid is standing on the bridge above Golemeg he asks for a summon back to the raid. Being my first raid with my Lock alt, and not really thinking about being anything other than a smart ass, I summon him above the crack behind the boss. Everyone got a laugh when he fell to his death. And the 3-4 of us that hearthed out had a laugh as everyone else died to boss agro. I still haven't lived that one down.

A long while back on our first attempt at throwing together a world boss raid, someone got promoted to help with invites, and the raid filled surprisingly fast. We're in an enormous guild alliance so it was not unusual to see new names, but when they started talking about Galv it became apparent - our raid was filled with an AV auto-raid-invite addon.

Our mages have taken a liking to putting up portals to Thunder Bluff at the end of raids. It is always fun to hear the people cursing on vent when they hastily clicked on the wrong one.
