Mages can cast underneath the water as long as they are right under the surface, if I go too low I can't cast at him
As for stupid things I've done, I've hit Ice Block instead of Water Elemental at the start of an arena match before

If your toon appears above the surface of the water, the spout will hit you. The issue is how far down in the water you are. Jump into the water from an island. Where you land, and the level you are at will get you killed. If you hit "x" once, you will drop and your toon will just just *at* the surface of the water. This is safe.
I hit "x" twice, just to be sure. It's not exactly a race fight: better safe than sorry.

I regularly jump forward into the water to continue DPS on Lurker while he's spouting. The key's not to stop DPS, but to be sure to be *under* the water, not just in it. Treading water gets you a spout to the face.
I still have difficulty jumping back to the platforms sometimes; I jump up, somehow don't land on the platform, land back in the water, my jump takes me under water for whatever reason, and then holding forward makes me swim under the platform. It really is absurdly annoying at times, I've also been bad at other jumps in this game, such as the glitch jump in Dire Maul West, the now-famous Blood Furnace boss-run, and so-forth. Even normal jumps get the best of me sometimes: Wailing Caverns just shouldn't have that jump, and I'd probably be the guy who failed at Thaddius every other week if I'd been in a Naxx guild.
This is an MMO, not a platformer. If you want me to jump, get me a Playstation controller and the latest Mega Man or Metroid. Is there really even a reason why we need to be able to jump in WoW aside from fun times in PvP? I'm all for movement fights, don't get me wrong, but jumping and falling to your doom just isn't good design for a lag-prone, latency-limited online game.

I know that much about Spout. I just don't see how the Hunter trying to shoot him while underwater was doing something stupid. = P
(I've been hit with Spout if I jump inside the ring and try to melee lurker while he's spinning around, so that may have something to do with it)

Well you see, I was trying to get high enough in the water to shoot him. I obviously didn't think the rule of "if I can shoot him he can shoot me" through very well. I also have something like 9k hp raid buffed (relatively new 70), so Spout just about kills me, never mind the boiling water.

I had a similar problem on my first night in SSC. I stepped backwards into the water for the Spout, but when I tried to jump out forwards, I found myself under an outcrop instead. Thinking I'd simply mistimed the jump, I swam backwards a couple of paces to free myself, but no, still something above me, back a bit more ... bit more ... I was getting very confused by this point; surely I should be able to hop out by now?
Well, somehow, my mis-jump had taken me underneath the island that had been behind my position. I had no idea that that was what was stopping me jumping out of the water at that moment, so I ended up swimming all the way under the island, coming out on the far side (to my great surprise) and running back to my assigned position just as the adds spawned on the island. That wouldn't have been a huge problem, except my computer had been locking up at random intervals all night (possibly a contributing factor to the original poor jump), and chose that moment to lock up again, while everyone yelled at me to get the hell away from them.
Wasn't my best night, that's for sure.

Actually, a lot of people in my guild complained that they were having problems with getting hit by spout whilst under the water that went away once they stayed at the surface. I was rather confused, but they swear up and down that they got hit underwater and never whilst above.

I always go underwater as opposed to staying on the surface and I've never been hit by spout, I've only ever been hit by spout with my head above the surface.

People get hit because they move too late, not because they are above or below the water.
Your position in the game is not "sync'd" 100% between your game client and the server. The reason people get hit is they wait till the last second to move and the server still thinks you are on a platform when the spout passes your location. Though on your client you are in the water.
I stand on the 1st island and step side ways into the water about 3 seconds before the spout is due to hit me. I don't move under the water (I simply press 's') and I don't move more than a few steps away from the platform. I'll cast a SB from within the water whilst the spout passes and then press 'd' and jump to get back on to the platform. I've never been hit by a spout.
If you find yourself getting hit, simply move sooner. The water only hits for 500/tick a few more seconds in the water will not kill you.

A paladin in tonight's Naj'entus kill died around 3 minutes into the fight, without being targeted by any spells since the last Tidal Shield. A couple of minutes later, he asks if Naj'entus has any sort of attacks on secondary aggro, because he got hit for 5k white damage. Everyone's wondering what's going on, until I decide to ask him the not-so-obvious:
<Me> Hey Paladin, you didn't have Righteous Fury on, did you?
<Paladin> I did, I have 3/3 Improved and it makes me take less damage

I wouldnt call that stupid, in fact very smart  . There is no reason he should be able to pull aggro from the MT in this spot. A pally with RF is only slighty more threat than other healers.

When spout comes, I jump in the water in the middle, continue dpsing Lurker and get moved back up automatically by Swirl!
And to add a stupid thing I've done.. pre-tbc I had an addon called "Prowler" which allowed me to shift between forms fast. For example it shifted me out of forms and into the appropriate form if I tried to cast a spell in wrong form.
We were clearing BWL and I was tanking Chrommie. Awesome feat for a feral, this was going to be my first Chrommie MT-kill!
We got him to about 20% and I was thinking: "this damn bronze affliction is annoying... I'll eat a sand and get rid of it". Yeah. It was a great idea until I realised I was in casterform eating sand with a bigass dragon-dog eating my head.
I was called stupid.
A dps warrior tanked him till the end.

I shoot him from the water entirely through the 'Spout' phase and I have never been hit by the damage. As long as your shoulders are under the water you are fine.
Perhaps the other chap was standing of one of the ledges as was not entirely submerged, our pally has a habit of doing this.
On to my 'not entirely stupid' moment. We were just learning Magtheridon and the try was going really well, i was assigned as a clicker and everything was going fine, he was on about 40%. Just after I clicked the cube, my wireless mouse batteries ran out. So the next 30 seconds was spent scrabbling around in computer bits box for another mouse, luckily i found a wired USB mouse. Plugged it in and had control with 5 seconds to spare before my click! Turned out that was the first time we killed Magtheridon, i make sure that i always have spare batteries near the computer now!
An older incident was during the MC days. We used to use Hunter Pet pulls for Shazz and Geddon, it was my first time pulling and i had successfully pulled the two trash packs so far. As i pulled the next pack, my /rs comes up.. "Pack incoming.", followed quickly by "erm.. guys.. Shazzrah Inc too". Luckily the raid was on top form that day and managed to kill both the trash and the boss quite easily.

This was long before they made the aggro bursts that puts a bosses aggro to the whole raid.My first time in MC and looking at Major Domo.The raid leader is going over the list of who is tanking what and when everything was set."Ed go start the fight and talk to the big guy in the middle."Goes the raid leader. Stupid me walked right up the middle and behold being ganked by the crew and the 39 raid members laughing over vent.

Having read through 16 pages of this thread and remembering waaaay back to MC it seems that my raid wasn't the only one with Shazzrah / Geddon issues, but it's still legendary for those who were there.
Hunter pet runs up the tunnel towards Geddon's room, whilst raid waits in Garr's room. As tank, I'm waiting where I can see up the tunnel. Conversation on vent goes something like:
me: "Uh, mate, you got an add with Geddon."
hunter: "I didn't see anything adding?"
raid leader: "ok, we can cope, what mob is it added?"
me: "Shazzrah...."
In TBC, I screwed up myself on Hydross. Late home from work, I logged in at the entrance to SSC and was instantly invited to raid to be MT for Hydross. I got summoned to raid by a lock, buffed in about 30 seconds and we were ready. Ok, I drink my invis pot, run in and start tanking. I'm just thinking, "Damn I'm taking a lot more damage than normal" when the raid leader calls, "Ok, wiping positions"
I'd forgotten to put my res gear on ....
Someone mentioned a while back about naming pets, well, when AQ came out, it seemed that nobody in our guild would ever bother setting their boss mod to NOT whisper when people got plague from those damn Anubisaths. So, playing my hunter alt, I got a new pet and named it same as some guy in another guild. Of course, first Anubisath, the pet gets plague and everyone in the raid whispers this guy "*** YOU HAVE PLAGUE - RUN AWAY ***" then they all start wondering why he's getting pissy with them!

I'm surprised I haven't read about tanks forgetting to put on their Ony Cloaks on the drakes or Nef  I know that was the cause of a few wipes in our BWL times.
I also remember learning how to AOE the supression rooms before Broodlord the hard way.

I used to play a rogue, long ago. I switched to warlock when I joined a raiding guild with my roommate, who plays a rogue.
We were in the break between Nef attempts, so (with his permission) I sat down and was checking out his gear. I switched between a few itemrack sets, oogled the stuff my rogue had missed out on, etc.
Nef spawns on his throne, so I set him back to his itemset labeled "BWL" (or "PVE" or something), and go back to my computer.
We did phase 1 as usual and suddenly my roommate dies. We couldn't figure out what had happened until we checked the combat log and yep, Shadowflame. Oops, I'd forgotten to put his Ony Cloak back on...

My guild prior to TBC had a feral MT from Luci all the way to Nef. Everyone thought we were crazy, but it worked for us!

The first time we were in kara I remember thinking "something named Maiden of Virtue can't be an enemy" and walking up to her to see if she had a quest...it didn't go well. In my defense, why is it we're killing the maiden of virtue, guests, and philanthropists for the first half of kara? Oh, and also when we were first working on aran we needed to go repair at the bs, so I rush down stairs and jump into the audience from the opera (we skip them). Being a rogue I vanish and continue down to moroes...not so for the 5 other people that follow me in jumping off the balcony.

I don't see any of these as stupid.
1) Sometimes you can't avoid getting feared by the striders. They spawn right on top of melee sometimes, and if the tick is at the wrong time, you're feared. I get feared every couple of weeks when I have a core and it spawns and runs past me. Yay running in place.
2) Our dot classes are *told* to DoT up Leotheras when he whirlwinds. It helps pull him back and forth in the middle as he resets aggro during the phase. Just don't screw up the transition back to non-WW mode (i.e. tanks L2P). Same goes for Hydross. Our locks and SPs have full dots, all the time. That's what Misdirect and Shield Slam are for.
3) You can't "avoid" the gusting wind very easily if you're a healer spamming heals. In fact, the only people who really can avoid it are ranged dps.
4) LoS to avoid shadowbolts? First, there's no shadowbolts on FLK ... secondly ... why is FLK in range of people in the first place to hit them with Cataclysmic Bolt (which is what I assume you're talking about). You shouldn't *have* to LoS ... just range them.

Wow, that was a very useful post. Ok, Captain Pedantic, I'll explain it in detail for you:
Ranged DPS and healers being feared by striders
Ranged leaving dots ticking on Leo so they pull aggro and get face melted
Healers not looking at the gusting winds and getting tossed up in the air so tanks die
People leaving dots ticking on Hydross when he changes forms
Why would you outrange Karathress when you can just LOS the bolt? And normally you would be kiting him across the room to avoid totem damage until the adds are down, unless you enjoy sitting on Spitfire while healing Karathress tank.

We had a Druid tank Nefarion as well (just for laughs) It went well though the healers spamed like hell when our Mt was stuck in catform but it was fun.
