Why would you outrange Karathress when you can just LOS the bolt? And normally you would be kiting him across the room to avoid totem damage until the adds are down, unless you enjoy sitting on Spitfire while healing Karathress tank.
He doesn't magically stop casting cataclysm bolt just because some folk manage to outrange it or otherwise avoid it.
The more folk avoid the bolt, the more it concentrates on a smaller set of folk. If you insist on having everyone outrange/los the bolt, your MT and his healer will eat all of them.
When Karathress does Beast Within and dumps a new spitfire totem on the MT healer, you gotta wonder if it's better to take a 50% chance of the cataclysm bolt hitting the MT (or his healer) or is it better take a 90% chance to instead have the bolt hit a hunter or a warlock standing off to the side?
When we were doing this fight, we put a very small number of critical people out of range (caribdiss healer and all the add tanks), leaving almost everyone else to take the bolts and thus take a lot of pressure off the MT healer.
Point is, while it's fair to rant against people who don't follow directions, I think perhaps your strategy isn't efficient. Also, different folks take different approaches - the strategy you've listed as being 'stupid' is one we deliberately chose and used to regularly 1shot this guy.
We had a Druid tank Nefarion as well (just for laughs) It went well though the healers spamed like hell when our Mt was stuck in catform but it was fun.
I've tanked Nefarian before through two druid class calls - as long as you have decently high dodge, it wasn't too big of a deal. It was definitely a lot of fun, although part of the contributing situation was that at the time we had no protection warriors in guild (MT/OT gquit because they got sick of the game and none of the other warriors wanted to spec DPS). I ended up becoming the de facto MT for BWL and AQ40 as a result.
Funny one from an early Magtheridon attempt, though not caused by stupidity on my part. This was on our second night of tries (we got him down to 25-26% on our first night), we finally managed to get a clean phase 1 and we were speeding through phase 2, everybody was clicking their cubes at the right time and we survived the transition into phase 3, by the time we had him down to 18% it was my turn to click again, the Blast Nova cooldown ended and I was sitting there waiting for him to cast. The raid leader calls out "Casting casting casting casting" but nothing was happening on my screen, I couldn't cast, nor could I see anyone else casting, let alone see Magtheridon cast Blast Nova. Yep, I was lagging. A few seconds later my client catches up and everyone is dead.
I felt awful afterwards, because we were so close to getting our first kill, but fortunately my connection has cleared up recently and I haven't had any lagspikes like that since. At the time, my ISP (BT, aka bunch of idiots who fail at running an ISP) was having lots of problems, I was having ridiculous pings at peak times, I know it's not really a story of stupidity, but it's amusing nonetheless.

7.Accidentally casting Misdirect in Mulgar and then hitting him, I was teased about that for weeks

A certain rogue trying to prove his pure PvP spec was viable for raiding, even after the good rogues laughed at him. Was a good laugh to watch the tanks outdps him.
Another completely useless rogue back in the day who had a collection of gear including 3 Gutgore Rippers (all with different equally useless enchants). His name was also so awful (Bonerboy) that we all reported him to GMs untill they made him change his name.
The same rogue decided before he jumped ship he would blow all his remaining dkp on Bijous and Nexus Crystals 5minutes before he left the guild. He got his name dragged through the mud almost no guild would take him, except for the retarded guild that made him a class leader.
After 3 hours of trying Leotheras people still come up with questions like:
"What was that black thing attacking me?"
"How do I avoid being mind controlled?"
"I can't kill my Demon fast enough can someone help me?"
Myself having one of the Magtheridon adds targeted while we buff up, hitting CTRL+T to open up another Firefox tab, oh snap I hadn't clicked onto my other monitor and T is my default auto attack, suffice to say I wasn't popular seeing as it was the 5th ninja pull on Maggy from me. The others included my dog hitting the keyboard, asking "Can I pull?" on vent and they said "no" but I heard "go", plus general tiredness and boredom. Yep I'm amazed they still let me tank bosses too.

Ranged DPS and healers being feared by striders
Ranged leaving dots ticking on Leo so they pull aggro and get face melted
Healers not looking at the gusting winds and getting tossed up in the air so tanks die
People leaving dots ticking on Hydross when he changes forms

People who think they know what is the best strategy and are convinced it's the only one, without doing their research or seeing all evidence to the contrary.
My favorite one:
People casting huge heals when Hydross changes forms.
You know that big heals happen to be crit heals (i.e. uncontrolable) and if the crit heal dealt more aggro that means that the tank was really low on HP in the first place (since the crit part of the heal wasn't wasted). Screaming at your healers in this situation will just make the ones among them that are easily cowed stop healing and thus concentrate even more aggro in the ones that are doing their job :P. Just make them move to the side where even if they pull aggro from the tank it won't lead to a phase change.
I tank Lurker in the water, which means that the Whirls knock me back up onto the platform, so I just run forward and back into the water. Once I did that and had my camera angle up a little too high so I went down too far. A tap of the space bar will fix that right? Jump up right into a spout. Hopefully the last time I ever get to experience that.
I also don't see what is so dumb about dots left on Hydross. This is why we tell anybody who might pull aggro (people with dots ticking, healers healing, etc.) to move over the line in advance. Making dot classes stop their dots cuts a lot of DPS.
Our MT will never use shield wall or last stand on important boss fights, until like 2% with a full raid alive. But on trash he will say in vent, "Whoops." That usually means he hit one of them.
Way back when the Prince was hard and you wiped to him and his trash regularly, there was that trash pull in front of him at the top of the stairs:
Tank: "Man guys, that was hard. We should be careful with this last trash. Hey Priest, can you go Mind Control the Star target?"
Priest: "OK."
[Priest runs up and casts Mind Control on Star.]
[Star is an non-humanoid mob, and thus is immune to Mind Control.]
[Star gets pissed off at Priest for trying to cast MC on it, and proceeds to beat the snot out of Priest.]
[Priest dies.]

Keep in mind this was AFTER we had done a heroic Steamvaults run where the same priest kept trying to cast Mind Control on the same type of mob in the pack pulls, and he kept complaining about "fluke resists" whenever he would fail and die during every pull. It turned out that the mob was IMMUNE to Mind Control the whole time.
You would think that after something like that, the priest in question would at least look twice before being told to Mind Control something...

After a wipe on Ossirian our trusty resto shaman ankhed to get the raid back on its feet and started to cast resurrection on one of the other healers. Strangely enough he disappeared after finishing his cast. Much hilarity ensued when he said he was back in Orgrimar..
Turns out his astral recall button was right next to the ancestral spirit one...
Way back in the dim mists of time (ie, right after launch) I was leveling up and was venturing into Gnomeregan for the very first time. This was a new and mysterious place for me, and I was particularly charmed by the elevators. Elevators! So cool! Up, down, up, down, up, down...
Now, I was a really really new priest, and I'd just picked up a shiny resurrection spell a few levels ago, and I just burned to be helpful. I often stopped to try and rez random corpses just in case they might be in the middle of a corpse run.
So, here I am on the lift and <lift goes down> hey, looks like someone fell down the lift shaft! Oooh! I'll help him. So I cast resurrection on him and <lift goes up> there all done. Gee, I bet he'll be happy to <lift goes down> avoid a corpse run! In fact I bet he'll... *splat* Oh dear, he just fell down the lift shaft again!
I felt pretty bad once I'd worked out what I'd done. I tried to make it up to him by trying to rez him again, but for some reason, he refused the rez...
One time we were fooling around while preparing for Ragnaros; getting pimp suits on, like the Tuxedo and whatnot. Someone started handing out Deviate Delights; I just had to took one, and was very happy as a ninja.
5 mins later, we pull Rag, and I notice my HP going down extremely fast, and maxxed out on something like 3-4K... managed to enter the fight with my Tuxedo on, could not notice it because of the Deviate buff
This one time I went with some guildies to do a quick heroic (?) Shadow Lab run. We met up, entered the instance and started buffing. As a Rogue I obviously don't have any buffs to give so I decided to mess around with the video options in the meantime. I had just bought a widescreen TV to use a my monitor so I figured I might as well try that option. Sure enough, after changing my ingame resolution my monitor went blank. No problem, I thought, it'll surely reset after 15 secs just like my windows video settings. Well, it didn't. So I had to run the repair.exe which by the way also resets pretty much each and every setting, including addons (all that with 4 impatient guildies waiting for me). I didn't want to keep them waiting longer than necessary so I ended up running Shadow Lab with my spellbook open and click-casting -.-
Tho there's probably an easier solution to reseting JUST the screen resolution... ah well

Our warrior (DPS one...) got his Thunderfury pieces finally, so we were heading to Silithus to summon Thunderaan. I got my Fraps running correctly for the first time, I badly wanted to capture the whole fight. Everything seems cool, Thunderaan gets summoned, we start fighting... like 2 minutes later, I notice that I'm not on the damage meters, at all.
Did not have SCT back then, so checking my combat log, I see wall of text of Immunes... Seems like the dumb elemental shaman tried to LB spam a storm elemental. I deleted the video quite quickly and hoped no one else frapsed it.
There was one time when me and the rest of the guild were waiting in Gruul's for the rest of the raid to show up. There was this one mage, I forgot what his name was though. He began casting and I was probably the only one watching, at first I thought he was conjuring water but soon I saw the blue beam of happiness, also known as Frost Bolt being shot out and hit one of the mobs at the first pull of the instance.

The raiders that were in the instance waiting/afk-ing died, and only like four of us managed to get out of the instance. Second most stupidest thing was that he died himself.

My main characters have always really been alliance though I've had a few horde alts. So ofc while leveling those I've done the classic ones such as running into Astranaar to get the quests there. Clicking on the alliance flight master in Gadgetzan and wondering why I was suddenly faced with the "Aggro" warning instead of the flight menu and similarly gone to repair at the dwarf outside BRM.
Currently leveling my Tauren Warrior in Outland so I expect to add a few new ones to that list...
On my paladin. I use "Q" for Blessing of Freedom. Mainly for PvP reasons. Can keep it up on a warrior with ease that way. When we do Karazhan however, I switch "Q" from Freedom to Blessing of Protection to BoP the Rain of Bones target when Nightbane flys at each 25% interval.
So we cleared Karazhan on Wednesday. I played my Priest on Thurs/Friday and didn't raid on Saturday. The guild cleared up to Vashj on Saturdays raid and Sunday was going to be spent completely on Vashj.
Sunday, pally chan. Styg, can you keep Freedom up on the tank? Sure no worries.
First pull. Tank has agro, dps starts and I go to put Freedom up on the tank. You can guess what happens now?
