Just yesterday: working on Illidari Council one of our shadow priests rezzed the pet of our hunter. As soon as the rezz was completed, the cat dashed towards the council, dying instantly and aggroing the council for everybody inside the room.
Turns out that if anybody else than the hunter rezzes the pet it will perform the last order given to it, which was attacking the target in this case...pretty hilarious :P
We've been farming Illidan for quite a while now without any major problems. A couple of weeks ago, I managed to pull something really strange off:
As always, I was tanking a Flame of Azzinoth. All went well and my mob died quickly and the raid turned it's focus to the other Flame. When the Flame was at 15%, an Eye Blast came sweeping right towards the tank and he didn't seem to notice it so I thought I'd save the day:
I start running, enable Shieldwall and I taunt the mob and drag it through the Eye Blast. Only problem was that I didn't hit Shield Wall, so I instantly died and accidentaly pressed release at the same split second.
All I heard from our raid leader was: "Chasy, I have no comment..."
Something funny that happened in Hyjal: do not talk to Jaina after killing Anetheron before you distribute the loot (in a master looter configuration). She will teleport all the NPCs to... somewhere, but not the players, then a ton of undeads come and just beat everything down.
We wiped and had to enter via horde camp, walk back to alliance camp and then kind of kamikaze to the boss corpse, trying to stay alive, distribute loot and retreat back to horde camp as fast as possible (mining some gem nodes on the way at least, so it was not all for nothing!)

Hrm, I was under the impression that they had fixed this bug. Perhaps that's still to come in 2.3.

We had that happen on our first Azgalor kill. While we were distributing loot, mobs started pouring into the area because someone had spoken to Thrall. Ended up having to train the mobs around the corpse so that the last winner could run in and grab their item.
Now I /rw 'Do Not talk to Jaina/Thrall!' after every kill even though most people know not to now.
Back in the BWL days I had a really early version of ItemRack running. Since we had good luck with two-handed weapons all the warriors in the guild I was in at that time had either Bonereaver's Edge or Untamed Blade. On phase 1 Nefarian we had the warriors (including the MT, which was me in this particular case) equip their 2h weapons and help kill the drakonids.
Anyway, I had two buttons assigned to switch to dps-gear and tank gear (this also was before the shield slam buff so everyone tanked MS specced). And since you can't switch out anything other than your weapons during combat I thought it was quite convinient to just push one button to equip my shield and sword when Nefarian was landing.
Turns out you get out of combat for a second or so the moment he lands, which made me unequip my onyxia scale cloak and promptly made me melt quickly after the first shadow flame. It was quite mysterious how my cloak kept getting unequipped (though people didn't believe me and thought I was just forgetting every single time).
It took about 3 weeks until I figure out it was Itemrack's fault

I remember a time in BWL where we were doing Ebonroc. Half of the guild never wore their Ony Cloaks for that fight... until one night when one of the Hunters somehow pulled aggro just as he was casting Shadowflame.
It was hillarious, even though I died a couple seconds later when I taunted and realized all our healers had turned to a thin, red paste. D:
I don't know if my guild was an exceptional bunch of potsmokers but while working on C'thun back in the days there was atleast one person per 3-2 raid nights taking a wrong turn and ending up pulling Ouro. It was fun in the beginning but it got annoying pretty fast.
And then of course, there is the classic "Hey you can see Ragnaros if you go into that hole" (the magma filled hole behind Golemag). Always worked on new recruits back when MC was the place to be.

It is for this reason I always had my hearth set to Cenarion Refuge (of course, I'm a Mage, so there's nothing spectacular about that).
I must have used my hearth about 50 times in our AQ days because of some stupid pulls. I saved a fortune.

I haven't done Hyjal yet, so this is to great help. Even worse than people ninjapulling the lever at 2nd boss in Heroic Blood Furnace. /slap
Our last Leo kill was a mess because of our hunter. The hunter was supposed to misdirect the demon onto our warlock tank when it split from the human form, but for some reason he cast misdirection and shot at the human form instead, leading to the warlock tanking both the demon and the human form at the same time.
Needless to say, they then went on to kill the rest of our raid one by one. Luckily, my ankh was still up, so at 1% I died, ankhed, stormstriked, wf procc'ed and Leo went down just as I died again. Our closest kill ever, with just a pally and a resto druid still alive afterwards...
This thread also brought up memories of TDC's very last BWL run. By that time we were all burnt out on the instance, already had bosses down in Naxx, and were only farming the zone to sell gear to random Hordies. After wiping to Razorgore 3 times (which is pretty lame for a guild that is supposed to be selling gear in BWL), we were prepping for the Vael fight and warning all the n00bs not to even try to pull aggro. We weren't worried about our MT losing aggro so much as the 4th or 5th tank not being in the proper order because some idiot decided to show us how 1337 he was by going all out right from the get-go. Anyway, we start the Vael fight, rogues and hunters go crazy on Vael because we know our tanks ain't gonna lose him. Then the first tank dies on cue. And the second tank is...our resident rogue badass (Mr. "I can take #1 on the meter in my sleep with no offhand"). We wipe. Everyone yells at douchebag rogue for pulling aggro. We go in again, get prepped, and start the encounter. First tank dies on cue. Second tank is...the same rogue. We yell at him again (this time with more profanity), go back in, and start the fight again. First tank dies on cue. Our second tank is...a mage. We all yell at the mage (with much more creative profanity). Then all the hunters (because we used to be really badass back in the day) yelled at all the other DPS classes to stop being dumbasses and stop racing on the DM for fucking Vael. So everyone gets sand in their nether regions and we try again. First tank dies on cue. Second tank...HAS AGGRO! Then loses it about 2 seconds later to a hunter.
At that point, everyone is going, "WTF GUYZ CAN U LIKE NOT PULL AGGRO EVAR JESUS KELLY CLARKSON!!!!" And we call the raid. And everyone vows never to set foot in freaking lame-ass BWL evar again.
Then about 5 minutes after the raid is over and we have refunded the n00bs their deposit gold, our second MT types the following in Gchat:
Tank #2: Uhh...guys?
Tank #2: So I was PvPing earlier today and before the raid I respecced back into Prot.
Random Guildie: Oh god...
Tank #2: And I kinda forgot to buy my skills.
Guild Asshole: MOTHER %()!@$(&ER
Tank #2: So the whole time in BWL I was kinda using Rank 1 Shield Slam...
Guild Raid Leader: YOU HAVE GOT TO BE KIDDING ME
Tank #2: Sorry about that guys...

-Actual EJ Forums feedback concerning Goreshot.
That reminds me of the time I mysteriously had 3 points left over after respeccing for Hyjal. After two days of struggling to do anything at all someone looked me up and pointed out that I didn't have defiance.
We kinda had a badish start to last nights raid, on teron after a series of wipes because an app get the ghost without knowing what to do, I get the first ghost and say confidently that Ive got it. Fast forward 60 seconds later, all 4 ghosts make it to the raid with 70% life. The reason for this was I recently re-did my macros and keybindings where it went /cast healing wave, and then /cast spirit lance, so while I was a ghost I was franticly hitting the spirit lance macro, but kept getting the message "you are silenced you cannot do that". We obviously wiped, and I was screamed at by everyone and their grandmother.
During AQ40 we made it to the twin emps the next week after our first kill and did in record time, we were buffing and tanks were flasking when the server crashed for about 10 mnutes. When we come back our GL at the time and myself KNEW that the trash had respawned but didnt want to say anything because
a.) that trash sucks having to clear once
b.) we didnt know if it pulled with the emps.
I have a small video of what happens to a 40 man raid when you're doing the twin emps and all the pre emp post huhu trash comes to play.
We had another mage use the sorceress dagger from MC to naxx because he was waiting for a "real upgrade" then lost the DKP bidding on the first splinter and quit the game.
we got several running gags by stupid actions, like back in bwl at vael: raid just entered the room, killing the goblins when vael starts yelling and killing everyone. first question was: "who talked to vael?" some new mage answered: "sorry, i thought he gives a quest."
our first attempts at maulgar. the mage who tanks krosh asks how far he can go without pulling maulgar. someone said till the vulcano. i remember me running back to the entrance.

We had something like that happen a while ago, except I was the last one alive.
This is what happened at the end:
Needless to say there was a lot of cheering in vent
Probably 2 months ago, we cleared first that week, and the whole time our fury warrior was bitching at me for low aggro, and I was sitting there going "shit, I mean I know I have been rage starved a lot but it shouldn't be different than last week, I didn't get any new gear." and so Illidan dies and we go to Hyjal. And he's bitching at me more, and trash is being a dick, and I barely am holding rage winterchill. Then we pull the next, and we're talking about it in warrior chat, trying to figure out what's up, clearing Anetheron trash and I just go "Oh.." and in raid chat go "Hey guys, fair warning, threat is going to be a little weak on Anetheron, and after that I have to go to Stormwind to pick up my awesome."
Cleared all of BT and the first 2 in Hyjal with rank 1 shield slam and devastate =x
Happened just now: Our guild got its first Leo kill, so the raid made its way to Vashj, saw the really cool bridge activation animation, and clumped up in front of Vashj's platform.
While we're staring at her, Paladin class leader activates engineering boots (the ones that add ridiculous amounts of runspeed for a few seconds and leave a blazing trail) and does a body pull.

I guess something I done was pretty stupid. It was back in AQ40 when we we're running the gauntlet toward Fankriss and we wiped at the boss. I died (or thought I had) right at the end of the gauntlet and disconnected from the server for a fiew moments.
Little did I know, I had just disconnected and when I logged back in dead I thought "Why are there no bugs in this tunnel?" All I heard on vent was..
"okay we're all here, lets buff up and give this another go"
"Is anyone else in combat?"
"yeah, whats happening?"
"RUN! RUN NOW!!"
"WHAT THE HELL!"
The MT had fraps on at that time and all you saw was a sea of bugs swarming over the small rise leading into the tunnel and everyone run around in a frenzied state of panic as every single bug in there moved as 1 tightly packed train of death. It looked like a scene out of Starship Troopers but Rico wasn't there with a nuke to save the day and only 5 people made it out
Lession learned, it's better to ask if it's okay to log after a disconnect in situations like that rather then just go ahead and do it!.
