There is a bug with that pack.. whenever shamans drop Elementals the entire raid's clients lag up for a good 5 to 10 seconds, unlag, and possibly lag again.
We've only ever seen it on that pack, and only ever happened when shamans dropped an Ele. Strange I know, just reporting what we observed.

This happened the first time we tried it, with 3 shamans. We have managed to get it working evey week since then, with only 1 shaman dropping the totem.

Doing Eagle boss in ZA, getting it to 20%. At that point I saw our healadin bubble and using heartstone. After he did so he said in /raid: I have to boost my friend and he leaves in 20 mins. Obviously we got wiped, had to find a replacement, which took a long time. Our guild is kinda short on geared healers. When we finally had a replacement we had to do the whole event up to the eagle boss over again.
Obviously the pala had a kick 
we need reliable people.
And figure this: Which Moron leaves a fight on a boss when it's at 20%?!?

Last night, we were running Shadow Labs, trying to get our main healer up to 70 (yes, we're _that_ new of a guild). We had myself, the holy priest healer, two mages, and a boomkin. We're one-shotting the place, all the way through to Murmur's room. First trash pull, I manage to face pull the second pack in the middle of fighting the first pack. I die, everybody else turns tail and runs for the exit. Our healer hits her Fade to temporarily put aggro on the dps, who are in front of her and thus don't have to worry about getting hit. About that time, I hear one of our mages say to the other, "Hey, hit Invisibility, it'll dump your aggro and we can make it out of here alive!" Predictably, our poor healer got eaten alive by voracious orcs. The swearing over voice chat that ensued was priceless.

In my first guild, back when we were learning Sulfurion, and the raid leader asked all the rogues to put mind numbing/wounding poison on their weapons, and our GM sheepishly replied he never did the poison quest.
Then few weeks later, after said GM finds out his wife, who also played and they shared an account, was having an affair with one of our hunters, logs in to find the hunter in our raid. He then proceeds to get invited in and starts going off on the hunter in raid chat and then gdisbands. I'll always have a special memory of Garr's room for that.

I stopped using my cooldowns on P2 RoS cause trinket+combustion+icy veins=dead fire mages. I still have to watch double crits as frost when a deaden is up.

We recently made some major roster changes to our guild... One of our two warrior MT's disappeared one day, another one /gquit a month later, our third warrior tank has to change shifts at his job. And our second prot pally decides to /gquit.
So we're running with two brand new warrior recruits. I have one of them tank Morogrim in t4 / t5 level gear. I think he might have had 19k hp fully buffed, if that.
He D/C's at 70%. He promptly gets obliterated.
Next attempt. He says he seems stable now. He d/c's at like 70% again. We were using 8 healers (Normally used 6 for this fight) so I call out on vent "SPAM THE MT till he gets back online!". For the next 90%, he d/c's for 20 seconds, comes back in and gets 2-3 attacks in and d/c's, etc.
I have to this day never seen any tank take so much damage. It was 90% --> 10% --> 100% --> 20% --> 100% --> 10% etc.
I don't know how but we actually downed him that attempt. Nor did I ever figure out how he didn't lose aggro. As soon as we kill him, he comes back online and stays on. I made him log off, go to the WoW website and change his password before he was allowed to raid again

My DI button is Shift + G. Apparently, It's a very easily accessible key to me.
There was one week where I managed to DI myself during buffs on SSC/TK trash pulls about three times. Then to cap it, we pull Vashj and I DI the MT. The best part? I was the only person that died. The warrior tank managed to sit there and wait out the debuff and somehow vashj just shot at people for a while and the healers healed through it. Being a stern raid leader, I decided that I did not deserve a BR after such shenanigans. There was so much inner conflict as I decided that.

I have 2, one of me and one of a guildie.
Me:
We were working on our second night of Archimonde attempts on Super Bowl Sunday (Sundays & Mondays being our usual progression nights). Needless to say I had been drinking during the game and was not in top form.
Before every raid I use my Engineering Transporter to see if I get an interesting malfunction, this night I got the race change one - my favorite. So I enter the first entrance and am changed to a ... female orc, my normal race. Ugh. So I run back out and in, trying to get back to my hot BE self. I have horrible luck, staying orc when I zoned in twice. I said "forget this, I don't even know if it will change again when I go through the second entrance" and went in. After that attempt, I thought "OK this time I'm going to get it" and went in and out a few times. Still had terrible luck, didn't want to hold up buffs so ran in to where everyone was already set up and got ready for the encounter.
Well one thing that I didn't realize until later was that running out of the instance deletes your Tears. Thrown in the air, go to click the giant button I have set up, nothing happens, I die, raid wipes.
Also the next attempt I cratered into a hill behind me as I watched the ground level in front of me. Didn't cause a wipe but it was still really stupid after what had just happened.
A few days later I realized that kissing Mojo has the same effect as zoning in when you have the transporter malfunction, and always change races that way in Hyjal now.
The other one:
A guild group is looking to do the heroic SH for the daily. We have a great group - some of our top DPSers, but no tank. Our Pally MT mostly refuses to do 5 mans any more, but suggests one from another guild for us. We start the instance, and we're ripping through stuff, thinking how we were the shit in our new tier 6 gear, and also "this tank is awesome, he barely needs any heals". Get to the first boss, kill him very easily, then someone says "Where's my badge?"
Duh, normal mode.
Immediately the leader position got passed to someone else but we all knew who was responsible.

I think the warrior deserves the mention for waiting out a 3 minute buff when he could have just right-clicked it off at any time (works even with funky buff mods like Elks) and gone back to normal. 3 minutes into Vashj and you should be thinking about the tainted spawn, not when the MT is going to come out of DI.

When you do bear boss in ZA, make sure no one is very close to the stairs. Otherwise you might share our experience from last night where our shaman got the random charge and got kicked outside, thus resetting the boss who was at 30%'ish.

Last night we were finally in Arcatraz for our third key fragment for Kara. We pulled together our usual team of two mages, a boomkin, a priest, and myself and find someone to open the door for us. Everything went smoothly and we got our key fragments, but that's when it all turned pear-shaped. We're sitting in the room with the first boss, and we decide, "Heck, while we're here, let's just do this one boss and see what drops, then we'll zone out to get our keys." We finish clearing the room of mobs and buff up for the fight. Our healer says she has to go AFK for a minute, and after she's gone, I jokingly say to the team, "Hey, I bet we can 4-man this guy, no problem." One of our mages apparently took me seriously, because the next thing I see is a big red fireball streaking across the room and hitting the boss square in the face. The surprising thing is that I actually managed to pick up aggro and we fought him down to about 30% before I kicked the bucket and we wiped. The not-so-surprising thing was how pissed off our healer was when she got back just in time to see us struggling in vain to stay alive.
[e] I had my own dumb moment last night, too. We're all flying around Arcatraz, waiting for our healer to get our first Kara key fragment group out so we can zone in. I'm with a moonkin and a mage, and they're playing chicken with the ground by seeing how far they can fall before hitting slowfall or flight form. I think to myself, "Self, you can do this, you have a bubble." Now Divine Shield was on cooldown, so I knew I'd have to use Blessing of Protection for this. So I fly up to a reasonable height and click off my mount. I see the ground coming up, so I click BoP, and... nothing happens. Splat. It was about that point that I realized I turned off auto-self-cast so I wouldn't BoP myself during a fight (it happens if my target gets destroyed before I can hit the key).

a) Last time we cleared ZA and was about to pull Zul'jin, I noticed that I have some items on "yellow" and I say "hold few minutes guys I have to repair" we didn't cleared hut inside instance as nobody needed the quest, so I mount up zoning out and repair.
My first thought on the way back to Zul'jin: "Oh I looted 5 Hex Sticks today so I can use it after boss down", of course I found only empty slots in my inventory.
b) When we had trial runs on karatress, and after some wipes one of our mages had to go and he getting replaced. At the same time we rebuffing, and warlocks placing healthstones. Instead of teleporting or using hearhstone mage that was gonna leave opening portal (On good distance from warlocks).
Minute later:
/raid For God's sake what 7 people doing in Ironforge?!
So "Click everything that shines" became a long-living joke of the guild.
c) Shieldless maintank on new Kael'thas pull tanking Sanguinar in phase1, not much to tell. But some people actually forget to re-equip their normal gear.
d) After 1st kill of Kael, whole raid rushed to look at Hyjal, and several people was so excited that took wrong reputation rings, it was solved by opening ticket, but still it's silliest things I remember for now.
P.S. Right after I wrote this post I logged back to game and getting a whisp from 65 level rogue. Do you remember that quest in Hellfire Peninsula for killing one elite demon wich awards you with blue trinket? It's called Cruel Intentions for horde and Overlord for alliance. So that rogue completed the quest and wondering what trinket to choose spell crit or defense one. He didn't even considered to take phisical crit one. After I explained why he was wrong, he brings his final argument wich nearly made me fall from my chair: "But I read on elitistjerks that spell crit makes my backstab hit harder"

We put together a 4 guildie 1 pug run of heroic SH last night. We're all chattering on vent, when the GM calls "PST for ZA invites" 45 minutes early "because we have enough people". Our MT /y's "OMG! MY COMPUTER'S ON FIRE" and we all simultaneously quit the group, leaving this poor lock sitting alone at the entrance of the instance, surrounded by totems as we all hearth outside.

To be fair, if you drop a Searing in the right spot on any threat-wiping fight, it is a HUGE help letting the tanks get aggro.
As for Stupid Things, yesterday in SSC we had no fewer than 4 people swim with the fishes. The funiest one? Our main tank jumped into the water to avoid a Spore Giant's death effect, the green pool of death. He'd forgoten that, since we were just barely entering the instance, the fish would still be alive

I love these little gems that we'll never hear the end of, and will forever torment us in-game and over vent. I signed myself up for the same abuse when we were in Kara the other night, near the Spell Shades in the upper reaches and, after I had shacked one, was sure to warn people in vent,
"Just a reminder, people, don't stand in melee range of the shackle, or it will start hitting you."
Yeah, that was awesome. Also awesome was the fact that our 25-Man raid leader was present, heard the whole thing, and continues to harass me to this day.
Unfortunately, I have very little ammunition to throw back at him, other than the fact that he once reminded the entire raid (yet-again), "Please do not stand near Thaladred...pretend you are next on his list!"...at which point Thaladred promptly turned to him (while in melee range) and one-shotted him.

I wasn't in the raid for this, but last night while my guild was doing Rage Winterchill trash one of our tanks died because many people were AFK eating dinner or something like that, anyhow, all the mobs went to our druid tank who then decided he needed to get more aggro by barkskin/hurricane. Pretty strong combo for tanking.

Our 2nd or 3rd kill on vashj was done with our OT completely hammered out of his mind. We wiped 3 or 4 times because of him, One of which he decided we were taking to long to buff and ran in and started the fight lerroy style. The next day he logs on.
OT has logged on.
OT "I was so trashed last night... i think i finished 2 bottles of wine before i blacked out"
Me "..."
OT "So how was vashj last night?"
Me "... you were there"
OT "......................."

We had a shaman drunk (and I mean drunk) bid on an Eye of Sulfuron. The next day, around 11am, he logs and and asks in guild chat "WHAT THE FUCK, I GOT A LEGENDARY". The man was so excited, although it was dampened a little bit when he learned that he had spent every bit of dkp he had, and would be broke for a while.

Ok, so after 4 hours of Vashj attempts (and getting her to 3/4 P2 consistantly with adds finally going down in decent time), the raid leader issues a vote: Once more with Feeling? The raid answers Yes, and the countdown goes once again.
Of course, fatigue is bad for raid coordination, and so is frustration. MT dies about 10 seconds in, and the RL calls out "run over, get out!". Everyone starts trying to hearth out before she finds us to hit, about 12 people manage it
