In my old guild, working on C'thun phase 1 one of our ideas for how to get in his room was to form a line all the way through the tunnel with approximately 10 yards between each person. We tried this about three to four times. That was not a pleasant night of raiding, needless to say. Getting forty people in an even line is harder than you'd think. :> (In our defense it was before he was killable and the videos weren't out)
One of our Hunters decided to open on Vaelastrasz with an Aimed Shot he started loading just as the MT pulled, during 1-hour Vael. He'd been doing it every try, except this time he crited. Not as funny without the totally surprised tone in his voice on vent as we chewed through him though. We taunted him with it all the way through BC, despite being a relatively minor offense.

This post on our guild forum was the result of - quite possibly - the longest consecutive evening of raid idiocy in .. well, in a long time.
SSC Hydross Walk-To - Downfall
The raid/guild officer that posted this was so massively pissed that he stayed up until 6am the night after the raid just making the diagram in Microsoft Paint. This will probably stand as <Downfall>'s lasting contribution to raid How-To guides.

Last night, during a Kael attempt. We were doing really well (finally getting to p4 consistently) and we had p4/p5 explained, thinking we had a shot at the kill pass.
Then our Warlock tank for Capernian got his MD right as he went to click off salv, and well, he clicked off MD instead.
Needless to say a phase 1 wipe kinda killed the night

One of our shamans water walked a mage as spout came around... he thought he bugged out ("I swear I stepped in the water...").
We also had a warlock pull Ebonroc with an eye while we were looting Firemaw ("Umm... guys, why am I in combat?"). On a subsequent raid when almost everyone had portalled out I asked him if he wanted to leave and he replied "No, but you probably do."
My own personal best is looking up and seeing burning adrenaline with 10 seconds left when Vael died... I managed to wipe out 90% of my raid, it was awesome.
And stoneclaw totem aggroes channelers. We've never let our shaman live that one down.

When my old guild was progressing through AQ40, we still did our weekly MC run to help people get their 8/8 T2 and get our MT his second binding. The mood was always light and there was lots of joking around (BoPing the rogues and such) since we could essentially sleep through the instance.
One evening, our hunter goes to pull Baron while we're waiting in Garr's room, and after a moment we see "[Hunter]: Uh...."
Soon after, Baron comes rushing into the room, but he is followed by a lava pack. At first people are pissed (out of habit mainly) but then we realize shoot, this might actually make the encounter fun.
So we down the pack and Baron, and pulled the same way every week thereafter.

Back when we were actively running ZG, it'd always be decided that after killing Panther and heading to Hakkar we'd "sneak" in between the patrols and mob groups to get past the bridge to central island Hakkar himself is on. Needless to say, there was always at least one person who messed up and this would frequently lead to three mob groups being pulled.
The most beautiful of such events however was someone running right into the patrol, the Priest in the patrol fearing him, and it ended up aggroing basically all mobs in a 40 yard radius of the bridge. It wasn't pretty, I can tell you that. Usually we'd manage to kill the groups under normal circumstances, but that was just too much.
The stupidest part however? We still insisted on trying to sneak past these mobs every single week.

After having Vael on farm for a few weeks, we walked in and expected to one shot him like usual. We start the pull and everything was going great until.... poof all the melee drops instantaneously and it's a wipe. We're trying to figure out who exploded on whom, but everyone in raid who had BA were dead at the back of the room like they should be. Finally, looking back over my combat log, I see:
"Forgive me, Barov Peasant Caller!"
...
Needless to say, the hilarity of a Barov Peasant Caller wiping the raid outweighed the depression of the wipe. Though the tank that popped the trinket remains a mystery to this day

I remember thinking I would be a smartass paladin and bubble-jump down the elevator shaft in ssc, only to find out that the freefall is longer than the 12 seconds of holy shield. That could have gone better.

I can remember thinking being clever and jumping down there myself as well. I usually have good timing on using Divine Shield to protect myself from the fall damage. Unfortunately, I met the elevator halfway through the shaft. I then further mess up by asking to be rezzed, accepting the rez, continuing my fall, and being too shocked by not being on the elevator to remember to use Divine Shield. One of my better days.
I'm also extremely talented at forgetting which mobs are or aren't cleared in Karazhan, so I jump down, use Divine Shield to avoid the damage, only to land right in the middle of 2 packs of those Mana Wyrms. Ouch.
One of our warriors though, noticing the fact that I always jumped down to save time thought there was some special mechanic in place in Karazhan's library that made jumping down there safe. This might've been helped slightly by me ("It's perfectly safe to jump down here! Just watch!"), and he promptly jumped after me when I jumped. As you can guess, he wasn't too amused when he noticed me using Divine Shield before hitting the ground, and then /gloating at his now deceased body next to me.

Our Hunters used to take turns shooting Golemagg from upstairs and then using FD. Russian Roulette MC Style, it took us a while to figure out while they were always lagging behind while we were clearing the Sulf. Harbinger Trash.

Reminds me of the hunter in our raid that we used to "eyes of the beast" pull Baron Geddon. He had been in the guild a while, and had been on plenty of MC runs. This particular raid we set up per usual for Geddon, hunter sends his pet off , everyone goes into combat and next thing we see is Shazzrah entering the room followed by Geddon.
"oh shit"
Never did forgive him for that...

I'm sure I have more, but my all time favorite was about 3 nights into 4 horsemen attempts. Things are going well and we are starting to make our first real tangible progress, we have a really nice attempt and then suddenly 4 healers all drop dead at zeliek (we had our entire ranged group stood on top of one another at 40 yard range). I assume a healer got too close, and ask around, it seems not and eventually we find the culprit is a mage who, when asked, explains that he 'noticed the scorch debuff wasn't up so I moved in to scorch'.
I still find that pretty funny, though at the time I was just completely dumbstruck. I'm still not sure what's worse, that he wasn't aware of a fundamental element of the fight, or that he had done it over and over for 3 days without ever noticing the scorch wasn't up before.

Turns out you can pull Grobbulus when he's above the raid tossing slimes at people...
Warlock: I wonder if you can hit him from here... (how many wipes have been followed by this phrase)
Raid: Why are we in combat?
Followed closely by all the trash between Grobbulus and the first room.
Exceptionally dumb move on the whole raids part. Post-TBC we decided to go back to Nax.
"I wonder if we can pull Patchwerk without his trash?"
- Someone mentions something about the aggro difference between 60 -> 70 and we decide to give it a shot. God knows why we didn't actually think about this for more than 10 seconds before pulling.
Frogger - Paladins waiting for people to start dodging slimes, then just bubble and run through, causing them to explode and kill anyone running at the time.
Kael trash - We were working on Kael for about two days (4 trash clears) before we realized the 20k Frostbolt was a weapon proc from the Mages. We couldn't figure out how the tanks were supposed to live through that.

I can't speak for every Paladin, but leveling a Paladin does two things:
1) Makes you very aware of how fun it is to hit Divine Shield as you jump off something and survive. (This doesn't work in SSC because the fall takes so ridiculously long, but it's usually just fine.
2) Makes you a megalomaniac who's unable to accept that there are things that can kill you.
1 + 2 = "/w <priest> I was dumb again. Res?"
Because I should actually contribute instead of just sniping, the #1 stupid thing that I see during raids is people refusing to stop DPS after an aggro wipe. The trash before Gruul is usually good for at least one wipe (and although we've never wiped with just Maulgar up...we have lost fifteen people who refused to stop DPS'ing, to run away from him when he was whirlwinding towards someone beside them, or whatever (okay, some of our melee don't have good timers on the whirlwind, but they think they do). Ever seen a Maulgar kill that took five minutes because all but three or four DPS'ers died after all the other adds were down? That was sad.
Heh...Back in the MC days, right before our GM/MT quit, we were doing a clear for t2 + bindings + items that never ever dropped. Early on in the run, one of our tanks speculated that we could pull Geddon in the middle of a lava pack and survive. That same night, we're doing a lava pack when I see one of the tanks back up too far and pull Geddon. Chuckles and sighs were had by all. Yes, every single melee DPS'er died, even the ones with capped FR.
My capped FR did save me on one other night of crappy Geddon, though! Our MT was highly amused by simply standing in Hellfire until the last tick after he got a few pieces of t2 and could usually survive it. Then, one attempt, he ends up dying to the last tick at 30%. Geddon turns to my Rogue and I tank him for the rest of the attempt - we already had a running joke about me tanking, but it got kind of ridiculous after that.

One of the worst Scholomance runs I've ever done. Yes, the recap is correct, I did more than 50% of the group's damage, a hunter pet outdamaged the "tank", etc. And yes, I did say "I left the oven on," leave the group, and hearth.

Back in the day, when MC was serious business...yeah, we had some chronic problems with the reflective shield. A few people seemed to think that you weren't supposed to stop DPS when it went up.

It works just fine in SSC actually, the trick lies in using Divine Shield when you can see the ground, as opposed to using it the moment you jump down!
... Just be careful of the elevator.

Back in BWL we were learning Flamegor. We had 2 Hunters for most raids, we told them to Tranq Shot Flamegor when he frenzied. Somehow, they were incapable of this. They got away with it because we didn't realize that his Fire Novas occurred only while Frenzied. We thought that was just how the fight went, and he just melee'd faster while Frenzied. This was back before videos were everywhere, so our ignorance was kind of excusable.
We ended up killing him for a solid month in "nightmare" mode, spamming Fire Novas on the entire raid and Frenzied from 100% to dead. We worked out rotations for healers (all wearing heavy FR) to bandage, had other healers out of LOS HOTing the main raid healers. We had to slam pots, demonic runes and Night Dragons Breath every cooldown every week. He was brutal and we couldn't understand why everyone said Flamegor was free loot. Eventually we figured out what was going wrong...the officers were so embarassed by their own ignorance that we couldn't even flip out at the Hunters.

Not WoW-specific, but I encourage everyone to try a singalong in vent sometime. Long ago on a ZG raid we found out it was someone's birthday, so someone started singing happy birthday. Then a second person joined in once they heard the slightly-lagged sound, then a third a split-second after that and so on. It turned into a mournful, slow dirge that sounded so awesome we had to spend the next five minutes finishing it.
"Haaapppyyyy Biiirrrrtthhhhhdaaaaaay tooooooo yoooouuuuuu...."

On Naj'entus: "Throw? Throw what? What lance?!"
On Vashj: "Why am I glowing green and can't move?! What is a core?"
On Archimonde: "Hmm, I am getting mad rage from this Doomfire, time to pop Bloodfury!"
Archimonde again: "Oops, I forgot to get new tears after buying reagents..."
And again...:"I didn't turn my camera while flying and didn't see the World Tree sneaking up behind me..."

In a previous guild last summer, something similar happened to us. We had just started Lashlayer and were AoEing the assorted whelps when the warning flashed on screen about 30 seconds to First Breath. It took a few seconds for the magnitude of the situation to sink in, then horror and panic as there was nowhere to run to followed finally by acceptance of the terrible fate awaiting us all. Sure enough, while Lashlayer pounded on the tanks in the corner, Chromaggus, Flamegor, Ebonroc and Firemaw all charged through the gate and annihilated all and sundry.
We had some great times in Blackwing Lair as it goes. Razorgore always gave us terrible trouble and one time actually had one egg left and no deaths. The controller went towards it, carried on running and ended up back on the other side of the room while Vent rang with cries of anguish. Realising his mistake, the controller ran back over but that single few seconds was enough for everything to topple like dominos and by the time the egg was destroyed, over half the raid was dead (like I say, Razorgore was our bete noir, we spent more time on him than anything else in there, Nefarian included).
Oh and who could forget the time on Vael where, so desperate to kill him we even resorted to the Upper Spire Spellbinder buff and potted to the absolute max, just as the ready check went out, a hunter who stepped in to replace someone summoned his pet - which charged right over and started biting the rather vexed dragon's ankles. One 90 degree turn and wave of flame incinerating 35 people on the spot later...
Gotta love it.

I used to organize and lead AQ20 raids on our off days, Tuesdays and Fridays. Every week I was farming the skill books for my guild and after more people got them less would show up. We started filling slots with pubbies and every week when the trash was all cleared before Moam I would say on vent "Ok for you new people, if you dont have the quest for Moam just jump into the water and swim down to the NPC to pick it up."
How many times have I moved in to Sap only to realize that I have not clicked stealth?

No matter how many times "hug the Lair Brute" is said on vent there are always 2-5 people at max range. It's incredible how often this happens and with the same people, the RL singling them out each time so they know who they are. It is as if they do not have their volume turned on for these particular mobs, but then again it's also mentioned in raid chat.
