On netherspite I was holding the blue beam('lock) and ran into the red beam to avoid the circle of death, took a 180k hit from netherspite, luckily we still had enough dps to kill him.

Had a great time in Black Morass one week. We were getting pounded in there, which was ridiculous, as we had a season one rogue, Tier 4/5 warrior tank, Tier 5 Fury warrior, Tier 5 Resto shaman and a Kara/crafted epic mage on the adds. Taking the place not even a little bit seriously as usual, we had a couple of close calls where the tank almost went down and the adds were stacking up. We down the first boss not too much trouble, and up comes an Epic gem. Turns out I had the instance set on Heroic, and no one noticed. We were farming the place for the Hourglass of the Unraveller, and hadn't planned on killing past the second boss. We tried for the end after realising it was Heroic, but got swamped by the adds. We also hadn't bothered picking up beacons - which may have turned the tide for us.

Why would you not pick up beacons, even on normal? Makes the last 5 waves really fast if you don't need them.

One of our mages ninjapulled with a pyroblast in heroic Bot last night. He likes to cancel his casts at the last minute while waiting for the rest of our mana users to be ready for the next fight. This time he got a lag spike and smashed the satyr right in the face with a big orange ball of death. Couple that with the fact that we'd been trying to get him killed all night, and I think you can guess what happened.

So, we were doing A'lar and this was the end of the fight a'lar is sitting at 4% and we have two adds up. Right now people are just talking about random crap as everyone is sure we have the kill. Out of no where the raid just blows up and people go flying and we have half the raid die (almost all the healers and a tank), we end up killing him.
No one really knew what happened (more wouldn't say), until I was bored the next day and decided to check fraps. At that 4% you see our feral druid (add OT) just run from the side of the room into the middle of the raid with his add at ~3% health (all dotted up) to try to get to a'lar and get 1 mangle off on him. Needless to say we still poke fun at him whenever he tries to pull the "1337 tank" statements.
Another time (on a'lar again) we are doing phase 1 and out of no where we just hear on vent one of our tanks go "ughhhhhhh" followed by random person "why are we taking so many flame buffets". Our tank fell off his platform right before a'lar got to his platform and he decided the grunt would satisfy as a warning.
In my experience, A'lar is one of the easiest fights to become complacent on. None of it is hard and phase 1 just bores the raid to death and some simple mistakes can damage the raid.

We we're prepping for alar a few weeks ago on an attunement run. Heard a rogue say on vent, I wonder if I can shadowstep him.
A seconds later we were all in combat. Luckily there was a tank nearby the first platform.

Summoning people into instances has introduced a new and fun way to screw with people! You can now summon people directly into the aggro radius of a boss or trash pack.
Works well if you have a mage to set up a portal for your escape after the summoned person pulls the mobs. If you don't have a mage you can also hearth if you back up enough.

Speaking of Illhoof;
A few weeks back a few of my Guildy's/ Friends started out for one of our 2.5 hr Badge run in KZ. We all stack as close as possible and pretty much ignor Kil'rek do to all the AOE going off. The Arms Warrior at about 85% pulled agro off the tank bring him from 85-0 maintaining agro the entire time. He was off headset due to being on the phone and it wasn't until afterwards that he even knew that he was tanking. He goes I wondered why I had so much Rage... I hadn't thought about his Racial so I asked him where the healing debuff was from. He just started cracking up, he goes it probably didn't help that I used Deathwish either. We only take two healers a Holy Pally and myself so needless to say that was my first challange in KZ for quite some time.

Perhaps you've forgotten which thread you're in...
My turn. I'm the guy who always brings his A game. I know all the strats, I read elitiestjerks...I don't make silly mistakes. I'm the guy who just ran off platform at the top by Netherspite, plummeted to my death and never moved to bubble myself on the way down. Seems alchohol slows ones reflexes. Actually with the amount I had in me, I'm not sure I even had a reflex.

When on hyjal trash i equip the spore shield from sporeggar rep with a shield spike to help solidify aoe aggro a little easier and then swap it out for the bosses.
Only this one time Azgalor comes wandering up and i'm taking a tonne more damage than usual, so i go through the checks: Ironshield potion quaffed, demo shout is up, thunderclap is up, all is looking ok. Around about this time however i notice there's a little green poison icon winking away at me, luckily i don't think anyone else noticed.

I generally don't spend much time reading quest text. After the 2.4 bombing run daily opened up I saw something about killing demons in the dead scar and headed down there to do so. After having some difficulty with the pit demons (they would mysteriously drop from the 80% health or so I'd get them to to zero, fortunately) , I managed to kill everything but the sorcerers, unable to find a way to get to them, so I skimmed the quest text again and discovered it was a bombing run.

Same for me on my Paladin. I noticed the blue explosions going off around me thus killing the mob and giving me credit. The one time I looked up the Dragon was above me and I just assumed it was him helping as part of the quest. What finally clued me in was I asked in general why I couldn't fire my Arcane Flares off even though I was in the scar. The "kind" people in general chat let me know rather quickly.

That alone is enough to get you in here IMO. My roommate would always bitch at me about how he didn't know what to do for a quest, and 95% of the time the answer would be in the quest text.

There is a new loot option called "automatically pass" which is nice to boost 2 or more friends in instances without having to use FFA (so the lowbies have about an equal amount of vendor drops) or having to loot yourself. You even get to keep your share of the silver.
Apparently you have to switch it off again if you want badges in a heroic afterwards... >.<

Hmm? I had auto-pass on for raids and looted badges off those bosses fine. Your problem was something else.

Well, after boosting some friends in SM and joining a heroic group the setting seemed to have reset to "roll, dont pass". Yet I could only get badges after I clicked on it again. A bit like having to configure your chat every time you log in.

So we spend 5 hours wiping on Brutallus, getting the healing and tanking down, etc. After two attempts where we get him to below 20% before the enrage, people start wondering. Hm, we had him to 50% halways into the enrage previously, why can't we get him to 0% before the enrage now? People shrug it off, and we go again.
This would be a good time to add that we run an incredibly warlock heavy raid, with 5 warlocks (our top dps) and two shadow priests doing shadow damage.
And there was no curse of shadows on the boss for the last 3 attempts.
Needless to say, things went much better after that, but didn't down him.
Next day, we come back and get at it. One attempt at 1%, wipe. Second attempt we get him to 3% before the enrage is up, somebody screams on vent to the tank "Run away!" (to give extra dps time, by kiting). Well, run he did. Cat form, dash, aaaaall the way over to the other side of the area, brutallus at 2% behind him. We lost 10 seconds of DPS on the guy, and just as he turns around after killing the fleeing druid he does his frontal cone attack for 20k on everyone in range, killing all the dps. Another 1% wipe.
Two hours later we kill him. Start attempting Felmyst a little, and while they're talking on vent, since the raidleader isn't saying anything, I zone out, totally ignoring everything said, while spamming mass dispell on nearby groups. Suddently, we enter combat. "Ehh... so I can aggro it from the air with mass dispell, huh. That's... pretty funny?"

We got Felmyst down to about 25% before she goes into Phase 2 well within the enrage timer. On the second breath the person calls "Go top, Go top ... yeah this is going to hurt ..." We got her down a few tries later.
