Along the lines of bad DI's ...
It was a couple weeks after our first Archimonde kill and things were going along well towards 1-shotting the boss. A number of healers were air bursted or feared with their breaks down so our ret pally decides to be the hero and LoH me (I had just hit last stand/shield wall so I should have been able to survive until my healers got back on me) ... but instead of hitting his LoH button he hit DI instead. And then I did the stupid thing of clicking it off, thinking it was just a bop at the time not realizing that all my threat was wiped. So I got to die with the entire raid.

Did a couple of UD-Strat runs the other day, to get Large Brilliant Shards for my weekly supply of Brilliant Mana Oil (and also boosted my AD rep to Revered, so that's me Naxx-attuned). I'm not an enchanter, so I have to drag a guildie along with me with a deal that he gets all the greens and rep drops if he wants, I get all the Shards, and we roll off for rare world drops. It works out nicely; I get enough Shards to have four or five Oils made, the Enchanter normally turns over about 200g/run in Greater Eternal Essences and Illusion Dust.
Last night, for my first run I took in a Warlock. Apparently the memo has not yet reached him that Holy Paladins with 800+ spell damage, pushback immunity and the ability to AOE-pull undead are quite, quite able to solo UD-Strat. So while I'm busy dodging as many packs as possible to reach the boss, he's summoning his Voidwalker to tank for him. It of course pulls aggro, and he gets splattered by several packs because I am 100 yards down the road at this point.
Rinse and repeat this scenario for 75 minutes, culminating in him getting us both killed on the way out when he ran straight through every pack under the sun. I swear to God, I don't know how he does it. I've seen people die less on T5 progression nights than he did to lvl61 trash.

Our guild is still trying to field the manpower to break into 25-mans, so sometimes we have to take group makeups that are less than optimal. The real question is how much less than optimal can you get without being totally stupid. I should've known that line was approaching very quickly when I brought in one of our hunters to fill out one of our last open DPS slots in an otherwise well-balanced Gruul's raid. I told him he would be using Misdirect on a tank and he asked me, "Uhh, what's Misdirect?" That in itself wouldn't have been so bad, but after a couple of pulls (and wipes) the tank he was assigned to said he never got an MD and had to run in and shield toss every time. I tried not to laugh too hard as I asked him why he was clicking "ready" on the MD ready check if he didn't get his MD. Needless to say, we didn't make much progress that night.

Had a new app in for archimonde last night. So i say in vent like i always do for everyone to get their tears. He thought i meant Tier as in his new t6 glove token. So he said yes he had it. Not realizing what was meant, and me assuming he had his tears since he answered yes, he proceeded to die to an air burst.
One of our best wipes ever was while we were working on Vaelastrasz. One of our rogues missed the salv buff after a wipe. So he proceeds to ask on vent who was doing salv. When he realized he was out of range he decided to run to the paladin. Unfortunately the paladin was on the other side of Vaelastrasz. Seeing as the shortest distance between him and the pally was a straight line, that was the route he took. He didnt get too far before val turned and killed him, and us.

We're doing Lynx boss on a ZA chest run when I notice totems seem to be dying a little slow so I decide to help on them (which I usually avoid since I can only hit them with MB/SWD). I promptly die as, following my Mind Blast, I'm hit by Earth Shock, Corrupted Lightning Totem and Shadow Word Death all at once.
Of course I've done the mandatory SWD-on-Enfeeble on Prince, so I'm not really a stranger to dying by my own hand. Oddly, somehow I've still yet to die to SWD on Curator's Evocate or RoS p2 or p3 despite repeated attempts.
Similarly stupid, but less suicidal is my tendency to pop all haste effects I can in order to snipe rezzes that are already being cast to bug our rez order nazis. (Makes for speedy enchanting as well, but that's less stupid.)

We were in the middle of the usual Tuesday-night crowd of groups starting a fresh reset of Kara last night, and we happened to run across an ex-guildy paladin tank. We wave at each other, get to talking, and I'm looking through his gear, which is remarkably similar to my own. As I'm in the process of summoning my group at the stone to get the run started, the following conversation takes place:
Him: You do know that Agility doesn't do anything for your dodge, right?
Me: Uhh, yes it does.
Him: Haha, you noob, no it doesn't. Run your mouse over your Agility stat, it doesn't say anything about dodge.
I honestly didn't know how to reply to that without either laying into him (there was some bad blood there relating to him essentially stealing some nice tank gear from my co-MT and then /gquitting) or just plain laughing at him.

You weren't a real guild back in the BWL days if you didn't have this happen atleast once. Had a rogue do that, too, though for the less hilarious reason "you're on the wrong side". He later became an officer.

I have a standard Karazhan story. It was one of our earlier Prince kills. I was tanking and standing in the pre-designated spot so the rest of the raid could hang out by the door. The hunter was going to MD pull to me. We had a feral druid OT that was stealthed waiting in melee range to open up once I got the Prince.
Combat begins and here comes the Prince. I was only half paying attention until he seems to veer offline with me. I noticed with a small level of panic that he has the druid targetted. I babble something incoherantly wondering how in the hell the druid pulled aggro and then watched paralyzed as the Prince one shots the cat. I really was completely taken off guard but managed to grab the Prince.
The druid is trying to explain that he did NOTHING to incure the wrath of the Prince. The hunter chimes in "My bad. I misdirected to the wrong target." The rest of the raid is busting out laughing to the point that the heals are just a little slow in coming and I drop.
We still laugh about this story and the hunter will never live it down.

We had a hunter MD Illidan to one the FR off-tanks by accident, who was a feral druid in cat form, and he got one shotted. It was pretty funny to watch. Next thing we hear on vent is "oops...".

Playing our 2v2 games on Monday we went up against a warrior shaman team. The acted weird off the bat because they exited their starting area and backed into a corner. We burned down the shaman pretty quickly, finally drawing her out, but the warrior stayed put. I notice a lot of parry/dodge/block messages popping up when we start in on the warrior (still with her back to the wall). She's doing almost no damage to my rogue partner so we finally figure out she's qued up in her tanking gear (and probably prot spec as well).

Reminds me of a funny trick we played on the MT in our guild a couple months ago. He's sort of the stereotypical laid-back, likes-everyone, wants-to-help-whenever-he-can MT (thank god it's not the prima donna version of MT--those are annoying, though no less easy to play tricks on). Most of us are almost entirely PvE oriented, but we have a few players who maintain 2200+ arena ratings, and just generally know a lot more about the PvP side of the game than most of us, particularly Mr. Happy-Go-Lucky MT. So our main rogue one day invites me, the tank, and a couple others to a group. He tells all of us but our unsuspecting MT what he's planning in whispers. We all think it's hilarious, and agree to help out.
Mr. MT is up in Netherstorm farming herbs (which is hilarious to watch in its own right, as anyone who's ever had the pleasure of following around a prot warrior trying to kill mobs, while you /dance). The rogue tells the warrior that he just wants to try something out involving a prot warrior in an arena skirmish, something about buffs and shield slam and being hard to kill. I don't really remember. It takes a special kind of person to think he's actually being serious (especially this rogue), but what can I say--our MT is that kind of special. I hang out by the Arena guy in Shatt to lend an air of credibility.
A minute later, the tank asks the rogue, "Wait, what are you doing in Ironforge?"
Me: "Oh, he's just being him. I rarely bother asking anymore. He's probably just sapping people at the AH and running away. I'll queue us in a minute."
I think you know where this is going now.
After a short skirmish where the warrior had no clue what he was supposed to do (first time he'd ever been in an arena!), we get popped out into the BG section of IF. I wish I still had screenshots of the ensuing confusion, but alas, they are no more.
Instead, I shall leave you with another, much more brief, amusing exploit. The setting is Heroic Steamvaults. The cast includes a prot paladin (Tomina), an enhancement shaman (me), a mage and hunter (Ied and Undra), and a tree (Girubeyote). We got feared into another group, at which point the tree decided he was a seal, and would escape the naga by swimming:

I feel for you. I never manage to jump right anywhere. Never managed to skip successfullu on those BFD floors.Couldn't get to Thaddius from the adds' platforms. That bridge next to Dun Modr, with the quest giver on the other side? Never got to him. I keep seeing people hopping and skipping while all I can do is fail.

Does it count as stupid to skip Hydross and Lurker to go for a new first kill and then come back to Lurker only to find out that the platform packs actually respawn while Lurker is engaged? Resulting in a hilarious wipe thanks to fishes in the water during spout.

I use a lot of macros to do multiple abilities depending on what form I'm in at any given time; I have one that casts Swipe/Rake/Starfire, one that casts Lifebloom/Cower/Challenging Roar, etc.
On to Brutallus. I've been specced resto for Felmyst attempts the previous week, and have just specced back. I'm getting ready to taunt off our warrior, and I get a resist. I reach for the challenging roar hotkey, hit it... nothing happens. I start to panic, mashing it like crazy... nothing. Suddenly, Brutallus switches to me, slashes my group just like he's supposed to... I'm at a complete loss. Suddenly, I hear "Ok, he's on Oak... battle res Pohra then."
The warrior MT had DIED (due to healer switch, I guess), and I was second on aggro, so had received aggro just in time for the slash to land on my group. It's the first time I've ever heard where a tank death prevented a wipe.
Oh, and as for my Challenging Roar macro? While resto specced, I had added [stance:0/6] to the Lifebloom check (it had previously read [stance:0]) so that it would work in Tree Form. This pushed the macro 1 character over the 255-character limit, which caused the final line to read "/cast [stance:1] Challenging Roa".

2050, trying out 2v2s with a new rogue. We come up against a warrior rogue, and they focus me. I drop to about 30% in the cheapshot/kidney shot. I come out of stunlock and try to pop my 1750 health trinket, and it doesn't work, whereupon I realise I'm still in my shadow PvE/pvp gear with about 200 resilience and 2000 health less than normal.
However we still managed to beat them quite comfortably.
So I put on my pvp healing gear, we queue up again and face the same team. This time I do die, but my rogue finishes them both off

Timed ZA runs:
Our bear tank tabbing to a crocodile and feral charging it.
Me trying to get to the eggs to aoe as the bombs begin to drop on dragonhawk and getting owned by the firewall(or fiery wall of fire as it is now known).
Our mage trying to blast nova a scout and getting 2 shot by the mobs who haven't been picked up yet.
Ward himself suggesting he spec shadow for a timed ZA...sigh.
Our tank who has spectral tiger outrolling me on bear mount.
Our bear tank wearing his "unhitable" set of gear for duel wielding bosses without kings on lynx
A bit of Illidan dumbness:
Shadowpriests midflaying the boss 2 seconds before demons and being gibbed.
Me running through eye beam debris in a mad attempt to get off the grate in between phases 2 & 3.
Our Flame tanks (who were perfectly fine where they were) backing up into eye beams
MT backing up out of LOS of the healers with Illidan enraged at 2%
People not running out of the raid with parasites
And finally: People letting phase 4 tank be killed by demons
Hyjal:
Our prot pally (who is usually ret) not having taunt on his bar for the first set of ghouls and having 3 aoers die in the first 30 seconds
Tanks forgetting that Infernals spawn up by Thrall while the warlock who was trying to help thrall out didn't...
Healers forgetting to assign someone to the frostwyrm tank and having the tank be desperately healed by a ele shammy until help could arrive
Our mage raid leader forgetting what wave gargoyles spawn on
The entire Archimonde encounter
And last but not least: Our feral tank accidentally switching to cat form on Azgalor and a warrior had to gen threat like a madman to get aggro while about 5 dpsers died.

I haven't been doing any BGs for fun since BC started. But now that I've finally got my 4 pieces of tier 4 and Illidan bow, I took some time to do the old AB and WSG premades that I used to enjoy in the old days. Mostly AB, since it's way more popular. But today is WSG weekend. First thing I do when I log in today, get a few mates and sign us up.
Anyway, about half way during the game, I pick up the horde flag, make a dash to Silverwing Hold, and wait it out with a couple of healers. Now, both of them were night elves, so they shadowmelded next to the flag. Good idea, I thought, and proceed to emulate them. Guess what... Shadowmeld drops the flag... I only figured that out when we had ours returned...
