I've just gotten a new keyboard which I'm still getting used to. Every now and then I accidentally fat-finger and press the wrong key (I don't actually have fat fingers, it's just that the keys aren't quite where I expect them to be).
The other night this happened in the best possible way as I blinked straight into a flame patch on Illidan phase 2, instead of pressing frostbolt.
Also I spent most of last weekend levelling my warlock from 68-70 and then continually grinding honor in AV/AB. So of course by the time our Sunday night raid rolls around, I'm still in "warlock-mode", and not "mage-mode". Life Tap on my warlock is bound to 5, the same key on my mage is bound to Blast Wave. You can probably guess what I tried to do in the middle of our sheep farm during Bloodboil trash

Emboldened by our success with HKM last week, we decided to try to finish off Gruul before the reset. After a rather heartbreaking 9% wipe due to bad timing, we mustered all our resolve for one final attempt. At about 6 grows, fat-fingered me managed to hit the Windows key on my laptop keyboard instead of hitting Alt (I use a lot of Alt combos for pots/trinkets). Unbelievably, I lived through that one and it was a kill.

Hunter misdirecting Felmyst to his pet without actually realising is a good one. Or side-tank on M'uru cheering when Holland scored and losing one the adds of the new pack (blaming it on "thunderclap resist"). And how can we forget not turning off rp-walk (to get on the balcony for Twins) and blowing up the raid with conflag. ;D
My personal favourite is redoing my bars to get some better keybinds on certain abilities, moving my aoe fear out for disarm. Then after the reset I changed everything back because it looked messy; this Thursday on M'uru I try to disarm the first mob and fear the entire pack instead (obviously causing a wipe due to lack of time to kill them off) making the fury warrior (who was a trial) shit his pants completely since he figured it was him.

This is a fiasco involving a Sunwell trash farm...
We were just finishing up a 2-hour Sunwell trash farm. For those who don't know, the first three pulls in Sunwell are giant robot then two 6-packs of blood elves which are separate pulls but next to each other. The plan in this trash farming raid was to kill the robot and one of the 6-packs and then kill all but one of the 2nd pack. Then everyone runs out, the 2nd pack resets and repops, we run in and kill it again and again and again. At the very end, the master looter finds a healing ring on one of the BEs. People roll for the ring and right as he's about to give it to the winning druid, GIANT ROBOT ELECTRIC DEATH! The two hour respawn timer was beginning to take effect. There is some deliberation as to what to do.. several people are bored to tears from killing the same mobs for two hours and some are beginning to drift away. The druid wants his ring. Finally, a skeleton crew of remaining raiders decides to kill the robot then divert the now-respawned blood elves while the master looter runs in and quickly allocates the ring to the druid. We do manage to kill the robot. Next, we prepare to have the MT run in, hold ALL the mobs for a few seconds while the master looter (a mage) gets the ring. MT runs in, I heal the MT, mage gets feared before he can get to the corpse, everyone dies. OK, back to the drawing board. By now of course the second pack of elves has respawned. Somebody hatches a plan involving chain misdirects that should give the mage enough time to loot while the MT keeps something like 12 mobs busy for a few seconds. I can't even go into too much detail on the plan; it sounded very complicated in vent. So, warrior charges in, several people spam heals on the warrior, misdirects seem to work! All the mobs are running away from the mage! The mage has clear access to the body with the ring as he kneels over the body to loot...
...and pauses....
...and pauses some more...
...as all hell is breaking loose nearby. Either the MT died or some mobs got away from the MT, I forget exactly what happened in the confusion, but after about 20 seconds of putting our plan into action we've wiped again. We're screaming at the mage, 'why didn't you loot the ring!?'
He replies "I tried like 30 times, the druid's bags were full."

Oh man, I've done this one. We had our first foray into Black Temple on Sunday. I was on my protadin at first, since we wanted three tanks for the trash on the way to Naj'entus. After spending the first attempt in my craptastic healing set trying to heal and hitting the enrage timer, I swapped to my hunter for the extra DPS. After a much better attempt, I swap back to my paladin to buff everyone with Kings, as I'm the only paladin available at the moment, and we had a number of people barely at the minimum health with Kings. Swapping back to the hunter for the next attempt, I fire off my Misdirection macro and watch Naj'entus head straight towards my poor pet.
My Misdirection macro is set up to cast on my focus if it exists and my pet if not. I'd forgotten that logging to buff the raid had reset the focus I'd set on our MT. Oops.

I've read through most of this. I hope this is not a repeat.
Slave Pens - the shorctut dropping off the bridge doesn't seem to be much of a shortcut when somebody always falls in. To make it worse, you tell them to take their clothes off and drown so they can be rez'd back to safety and the nubs swim all the way to the bottom out of range! Just clear the trash down and forget the shortcut unless u know the group well.
I just thought of one more. Stop posting meters in gchat from a raid! If I'm in your raid I'll read it there and if I'm not I sure as hell don't care to see it!

I do something similar. I have a clique bind for alt+right click as chain heal rank 2, and occasionally as I'm spamming it, I'll hit tab to target the boss to check its health. Well, alt+tab is bound to something else entirely...

On my paladin the button left of my mouse is a seal/judgement macro, so when I'm about to enter combat I press the button and move(paladin for the loss) into melee range.
Now, I get on my mage where the same button is fire blast. The results we're...well let's say that I'm happy I play on a laptop and luckily I have a key here that Disallows wireless internet acceptance.....(read: Disconnects me).
Mshing the wrong password on your logon screen is a nice one as well. I tend to change my WoW-password a lot, I'm currently wearing a tinfoil-hat and have my firefox equiped with noScript to give you a clue.
Now I change my password from dungeonlords137! And to..well Let's not broadcast that one, at any rate, I kept mashing the old one and I was so freaking angry and afraid at the same time.
The 1337kid inside me got the better of me: **Zomg i gut hakket bastorts china giays lulz I fukin'h8 tem lulz**
....
Erhm, wait....

First night for a new Prot Pally in BT we decide to test his gear on some trash. He had done ok on the AOE packs before Supremus. We have a hunter misdirect one of the big doomlord demons onto him as we clear the road towards Supremus.
Five seconds into the pull, the hunter has aggro and feigns. The demon runs into the raid and we are all wondering what the hell is going on. One of the other tanks pick it up and we kill it. The pally pipes up "Sorry about that. I bubbled." Two or threee people quickly add, "Well at least you didn't hearth..." Poor guy will never live that one down.

While this is true, remember that gold-sellers are also into account hijacking. If they happen across a 70 on an account they hijacked that they can't squeeze gold out of some other way, sending them into Kara to spam would seem plausible.

I pretty much managed to do a similar thing as protection spec during a ZA speed run. I had warned the raid in advance that my tanking skills was fairly limited as I hadn't been playing the spec a lot, but we were short on tanks so we gave it a shot anyway.
We arrive at the eagle boss, with a fairly easy trash clear. When we engage, I manage to hit the my divine shield hotkey (my judgement hotkey is v, shield is shift+v), and the boss kills pretty much all of our dps before I finally realise my mistake. We wipe, shit happens. Everyone hurries back, telling ourselves that we can at least make the best of it and go for three chests. I engage the boss, but the boss suddenly starts going for all the dps. Turned out I had forgotten to turn on Righteous Fury. That was some speedrun.

The first time we should have killed Felmyst was just an utter failure. She's landing for the last time at 22% or so with everyone in the raid alive. It would have been an easy win with out a 1% flight phase. Its called in vent to BoP our shadow priest (also one of our mass dispellers) because he's high on threat. Felmyst goes crazy 1 shotting all 3 hunters 2 warlocks and our spriest before getting back on our tank. Gas nova goes off and wipes out our melee group because it was the spriest's dispel group. Leaving us with pretty much 0 dps left alive.
The pally BoPed the Tank.

Due to general urge to max dps (also known as laziness) is my hunter running with a /assist /cast MD /assist macro, to reinforce the tanks aggro.
Karazhan run about a year ago, when the trash before prince actually still was considered to hit hard, a mage gets a bit too close to tanks threat, so I MD.
A second too late.
I do run with a /raid casting misdirection on %T line in that macro, so the whole raid got to know it, and laughed their ass off, since with his new arcane spec, and with no aggro reduce he'd been dying like a fly for the whole run, that was his 11th death in one clear.
EDIT: actually, know I think about it: half this thread consists out of paladins, the other half out of hunters.
MD is just too powerful an ability to be given to hunters.

My rogue class leader having to afk half way during our first Kael'thas kill due to some car insurance person stopping by IRL - we never let him live that one down.
One of our resto shammys managing to die 3 times to air burst (ankh & combat rezz).
The good old paladin reflex when targeted by some boss ability: bubble at Solarian before it got fixed.
Oh, and of course throwing the core to the strider kiter is always good for a laugh...

We were doing a PuG Vashj run the other day. During our best attempt yet, the last tainted elemental spawned, we killed it, and went to throw it up and finish off phase 2...

The DPSer saw Shaman T5 and in a rush to finish the phase before another Naga/Strider spawn assumed it was just another healer. Too bad it was the Elemental Shaman kiter.

When I spec Ret for fun or for arenas, I usually swap my rank 6 Consecrate and my rank 1 Consecrate because I have absolutely no mana regen and I never run anything seriously damaging enough to get mana back from heals. I went Ret for a little while to play around with the badge libram and some random Kara epics I had picked up, testing out my leet dps and so forth. Later on, our ZA crew was online so we decided to hit the first two bosses and try for the timed chests. I dutifully respec back to Prot, slap on my awesome, and fly out. The run goes fine, though I seem to be a bit weaker on threat than usual and even had a mage die during one of the eagle swarms on the way up to the eagle boss. Nobody said anything, though, so I assumed I was just seeing things. We decided to do Nightbane after we got out of ZA, and during the first attempt, our bear OT pulled aggro on an air->ground transition... 15 seconds after he lands. Now I know something's not right, and during the wipe recovery (the aggro pull cleaved half the raid), I finally noticed that I had managed to do the whole timed chest run AND 25% of Nightbane with rank 1 Consecrate.

No, because tears don't incur GCD while Blink does, and Blink is best used for getting the hell back or braving a Doomfire to decurse melee.
besides, if you're considering a chain-of-contingency at all, you're not the person who forgets Tears.

Last night my guild was doing yet another farm run of Hyjal, since we were late kill Illidan and need to farm like crazy for SWP. We just one shotted everything up to Archi and took somewhat of a small break while our various members went to respec healing/ret etc. Half the raid was practicing their non-crater jumps off of the practice cliff when my boss mod started going berserk. After some confusion and frantic vent speak, the entire raid was dead from 99k finger of deaths (with the exception of the few who were respeccing.) After some deliberation as to what caused this slaughter one of our warlocks who had just came back from afk said, "Oops I was on autorun, thought I was on autofollow" Never ceases to amaze me.

A lot of people have amusing fat-fingers stories, where we accidentally hit one button instead of another, and something completely bad (or at least funny) happened. My guild sort of took this to an extreme, though to be fair, nothing bad ever really came of it.
A long long time ago during a Kara run, I'm running out of one room and into the next and pick up my totems using Totemic Call. For some reason my totem timers didn't reset, though. Someone on vent says, "Alex...? Did you mean to do that?" Turns out my Totemic Call and Bloodlust buttons were right next to each other. I sheepishly admit to this over vent, and the other shaman in the raid says, "Crap. Um, hold on, changing some key bindings around." Turns out, she had done that a couple times too. Over the next couple days, we discover that almost everyone in the guild who had a 70 shaman had those two buttons next to each other, and most had accidentally Bloodlusted between fights.
Not quite as bad as the fact that almost every mage I know has, at one point in time, had Polymorph and Blink right next to each other. Of course, that lesson I learned the hard way on Domo back in the day...

I am a hunter. The name of my pet is "Hotpocket". Here is the story behind it.
Back pre-BC, I played a shaman. We were working our way through BWL one day, and were on Vael. We were all assembled, buffing up and getting ready for the pull. Suddenly, one of our hunters shot Vael, who then proceeded to slaughter us all. We asked for an explanation for the shot. As it turned out, he was biting into a hot pocket, which burned his lip, causing him to drop the hot pocket, knock his mouse off his table, and activate his Autoshot with Vael targeted.
Fear the Hotpocket.

Worst reason to not use blink I've ever heard.
Who cares about the GCD in this situation? With blink you get back to where you were in an instant, using the tear means you'll fall for a while (actual fall time depends on how close to the ground you dare clicking). So "losing" the GCD is nothing.
Only time I use the tear is if blink is on cooldown. Blink should always be the first thing to try.
