Along these lines, while clearing the trash behind the Opera stage in Karazhan our GM (playing her alt rogue) decides to run forward to Barnes' room for some reason or other, completely forgetting to stealth. She pulls a pack + patrol and vanishes, but her boyfriend (our raid leader, mage) has followed her into the room... and runs back where the rest of the raid is sitting drinking. Wipe hilarity ensues. :/
Why did the raid leader follow? "I was staring at your ass." Pervy gnomes ftl.

Our guild's proudest moment:
WarcraftMovies.com - World of Warcraft Movies
We were in the top 100 ouro kills worldwide (that were tracked on the R&D forums), this was one of our "learning attempts" where we tried to find out exactly how far the sandblast would go, and how burrows worked.
It starts out a bit slow (3 min total), but get's pretty good. Warning, def not safe for work language.

On our would-be first Onyxia kill back in '05, our first time stably reaching phase 3, one of our Paladins accidentally cast DI on the MT. (In an amusing reprise of this timeless guild raiding story, it happened again during a Maiden of Virtue kill two years later).
Once when Nefarian was landing, the MT picked him up, and then the boss suddenly starting running around killing healers. After the wipe, MT says "you know, Flask of the Titans and Limited Invulnerability Potion look the same in your inventory."

Pfft. I didn't *somehow* manage. After opera, I tried, saw that the castbar started and I had LoS to the audience. I cancelled the cast, innocently asked on vent "So, would it be bad if I threw my shield at the audience." The (likely not serious) affirmatives were all I needed. Hilarity ensued. At least I provide my own Benny Hill (Yakety Sax) music.
It takes the audience almost 45 seconds to make it around to the Opera stage. Interestingly, some of them stop and attack you through the floor from the area beneath the stage.
Fun times. Fun fun times.

Yes, if we're having a particularly easy night, and everyone seems in good spirits, beware Yakety Sax. It likely means something absurd and amusing is rapidly heading your way. So long as everyone is laughing their butts off, its all good ;-)
I will admit to not-funny accidental wipages. Guilty of discovering that the summoning of a Steam Tonk is an aggressive act when it was next to my elixir in my bags and I clicked the wrong slot during Magtheridon. Also guilty of getting seriously confused on my first time tanking Prince and, during phase 3, targetting myself with my BoP keybinding instead of my Cleanse keybinding (they don't even share any common keys *brain fart*). Prince charged and killed the warlock, who then asks on vent "Xan, why are you bubbled?".

We were doing a ZG run and had cleared to Hakkar. Our tank was one of our main warlock's alts. 3 of our other warlocks were in the raid (Proper raid makeup?? Wha??) and we had been bullshitting (read: conspiring) in the lock channel throughout the run. At the very end of Hakkar, the alt gets MC'ed. And subsequently hit with triple CoD's. What made it even better was it only showed up as one on his debuffs, so in the middle of "Haha, the healer's wont let me die to tha.....oh."

Edit - do other guilds' warlocks often try to get soulshards off themselves on spell reflecting mobs, or at least have tried?

I have actually been trying to do this ever since we started on the trash mobs leading to Leo/Morogrim, but as far as I can tell, it gets Reflected and that's it.

I have to agree, this is briliant. For my own stories, first a couple of amusing ones.
1.Was in the first room after Twin Emps clearing towards C'thun. Our main tank gets feared, runs into a column, falls through the world to his death.... in Westfall.
2. Naxx instance servers crashing and booting everyone out to a point below the Alterac Mountains where we all fall to our death forming this huge column of dead bodies. Others had it do the same but dump them in The Hinterlands.
3. First ever attempt on Leo. It goes predictably horrible during the demon phase so we're wiping it. A hunter that got Inner Demoned feigns death until we're done dying. Leo resets but Inner Demon triggers after this mind controlling the hunter. He runs up next to Leo and obediently stands at his side. Eventually, we had to pull him and kill him to get rid of it. I think he came by himself w/o Leo or the adds but I was laughing so hard I'm not sure.
My Idiocy
1. I had been farming low level mobs so I'd downranked SW:P to rank 4. I didn't realize I was still using rank 4 until two full raid nights had passed. Despite wondering my dps was lower than I thought it should and going through both nights WWS parses, it wasn't until i was pvping for the weekend I realized my error.
2. First Karathress kill, we'd just killed the priest and heading to Karathress when he drops a Spitfire Totem. The totems are immune to everything but Mind Blast and SW: Death so it's normally not worth trying to attack them myself. Since only a couple of people were in range of the totem yet I figured I'd help kill it. Here's the log:

Others
1. Killing time after a raid night and going to do Onyxia with about 25 people. We were finishing buffing just before Ony with the normal mindless hopping around. A warrior misjudges where they are and jumps off the little bridge right into the whelp pit, quickly getting swarmed. Of course a couple healers try to go heal him of which at least one manages to fall in as well and then the whelps come say hi to the rest of us. Oddly enough, none of this ever aggroed Onyxia herself.

putting UA on mindcontrolled people is always a nice retard check for the healers. had a couple of (5man) wipes due to it (priest getting crit for 6.5k or something like that), but well worth it since we were laughing our asses off.
for a nice little variation, the double retardo: reflective shield goes up, warlock doesnt notice, casts UA on himself, priest doesnt notice, dispells it out of reflex.

Famous story from my guild:
Doing Ebonroc, and this one tank keeps on dieing horribly. Clearly no one was healing him. Healing assignments were given out, gone over again, and still the tank is eating it, bad. The raid leader is chewing out the healers and the tank is, obviously, pissed off. Same thing happens again for 2 attempts, and then all the healers start laughing. One of them finally pipes up: "Hey, chaddius, nice Ony cloak man. We noticed a few wipes ago."
Everyone had a good laugh.

Few things we've had happen:
Back in the days of MC, we were buffing up everyone (and those that remember the pains of 5 minute single target pally buffs will sympathize here), yelling at people to put on FR gear, use FR pots etc etc. We finish buffing and call to move to positions. At this point the group starts splitting up but theres 1 paladin running in a straight line at Rag while hes yelling over vent: "Im lahgggggggginnnnnnngggg!!". I don't think I've ever laughed quite so hard at a wipe before.
Apart from that we were doing the little grobules that litter the path to Patchwerk, when one of our hunters managed to tab target Grobbulus and shoot him, bringing him and the whole of the abomination wing trash with him.
We were also going for our first Loatheb kill (after doing full world buffs, pots and flasks etc etc), and stressing the need for the tank to get agro before doing dps (or just holding off altogether until you get the spore buff). We pull and before the MT even hits Loatheb, a fury warrior smacks him, gets agro and dies, followed by a paladin with severance. After wigging out the door and managing to reset him and salvage what was left of our world buffs we try again. We move into position and just before we pull, a hunter starts talking on vent. While she is talking however, we hear the sound of a bow being pulled taut. The hunter had managed to hit autoshot button while talking and pulled Loatheb, purging the rest of us with world buffs.
At this point the raid leaders just logged out :P

We had a rogue throw a knife at one of the Ushers beside the audience while we were waiting for Barnes to finish blathering on. R&J without a rogue to kick Eternal Affection -> painful (the rogue got ported to the usher and splatted, then the usher deaggroed).
Ironically, the same rogue snowballed Noth after we were running back on a third-down wipe (at 20% or so I think) - we were gunning for a kill on the next attempt, then as we were buffing/potting up and the raid leader was exhorting us to greater heights we hear "DIE, TRESPASSER!" ... TS went vewy vewy qwiet, then the raid leader asks in a very tightly-controlled voice "alright, who pulled?".
Also, BoPing the melee emperor tank as he's running in ... not such a good idea.

From a Magtheridon attempt:
Recruit: "Hey, one of the tanks is using a two-hander"
Raid Staff: "Um, none of the tanks are two-handed"
Recruit: "I saw him! We'd do better if he used a shield"
Tank, very calmly: "That's Thunderfury."

I have a theory... in WoW there are "jumpers" and "non-jumpers". I am personally a "non-jumper" because I like to listen on Vent and get my head in the game but I have noticed that people who are "jumpers" seem to perform worse than "non-jumpers". I really don't have any specific numbers or anything to back that up, but it's just the way it seems.
Also, I've also noticed that the "jumpers" are usually the ones responsible for ninja pulls.
My one other pet peeve... that god damn Piccalo of Flaming Fire. That thing pisses me off to no end.

Threw a Rejuv on my rogue friend as we were about to cross the second rope bridge in LBRS. The rogue in question promptly falls off the bridge and lands amidst the orcs beneath. They slaughter him, which is fine, but my Rejuv ticks while they're busy slaughtering him.
We wait for him to run back when I notice a movement in the lower right hand corner of my screen and realize that I'm still in combat. The orcs down below have healing aggro on me, and they're charging upstairs with the entire troll room in tow.
We didn't make it out of the instance.

I remember hearing about an ebonroc attempt where the tanks were completely unable to manage the debuff and about 10 minutes into trying to kill him the healers had a mage open a portal then left.

Wipe on Frankriss. Rogue vanishes. Rest of the raid dutyfully runs up to Sarturas room. We just started buffing up when we got an 'Oh shit' on teamspeak.
Somehow he removed stealth and aggroed frank. Thus the whole fucking gauntlet got linked to us. That was a HUGE train of mobs. It looked especially funny cause the models of the mobs were inside each other, so it looked like one gigantic wurm running to us. Eventuall the 200-300 bugs split up though and the whole room was full.

Speaking of Ebonroc, I used to die to him nearly every single time we fought him. Our tank positioning wasn't the greatest, but it mostly worked. Occasionally, we'd get kind of glitchy movement from him - and that usually consisted of him floating overtop of me right before he shadowflamed.

From a friend:
It was back in the truly good old UBRS days and he was pugging UBRS with his hunter. This being sheer hell, deaths were frequent, but somehow they managed to continue despite the obligatory wipes, so all in all it was better than many PUGs. Now, it seems that some of the members of the raid liked to duel when waiting for mana and buffing &etc, and this is where the story turns positively unreal.
After clearing those hellish trashpulls next to the Beast and killing the Beast itself it was time to cross the bridge and clear patrols from General Drakkisath's chamber. For some reason, once they reached the bridge, the two priests in the raid decided to duel, and one promptly mind-controlled the other over the side of the bridge to splat somewhere in LBRS. The other priest, looking down after the first, managed to step off the bridge himself with the same ultimate outcome.
Meanwhile, trash had respawned. Lacking a warlock to summon the priests to the rest of raid if they corpseran, and the raid lacking the power to fight through the respawns to the entry without healing from the priests, that particular UBRS attempt promptly ended.
