About Hakkar, I'm not sure. I think we're mostly considering him to be small fish because of what I like to call the "Zalazane Syndrome" (i.e. "he's only level 10, how can he be powerful?"). We fought Hakkar in one of the easiest instances in the game a long time ago, but all the build-up leading to him does try to present him as a threat to the whole world - while he might not be an old god himself, it's not too far-fetched to picture him being related to them in some way. And he still, still, still has the best line in all of WoW's bosses - which, incidentally, is directly related to the themes we're discussing here:

I agree with this entirely. People are downplaying Hakkar because he is a level 60 20 man boss that can be solo'd now. If you look at the first time he entered the world he was close to wiping out the troll empire (largest empire ever on Azeroth) and it took their combined might to seal him away. The trolls were ancient enemies of the Aqir too, if Hakkar is either an Old God, or more likely an agent/spawn of one then it makes sense for him to split the Troll empire with massive sacrifices and a growing demand for blood causing a civil war.
He also has a ton of connections with the Emerald Dream via the Sunken temple and the greens there that guard it. I really don't think it is a huge leap in the slightest. Eranikus speaks of serving his God several times, after spending so much time in the Sunken Temple with Hakkar it isn't a stretch that he is what corrupted him. As for being a small time player, he warrented the personal attention of Ysera and the green dragonflight, bearing in mind lore says Ysera rarely leaves the Emerald Dream, to come and smash up his temple in SoS and then guard him for thousands of years.
Also who says an Old God has to be universaly big/blob shaped? The thing in Darkshore certainly looks different, and anyone familiar with the C'thulu lore would know that they are pretty chaotic things. Also don't forget that one of the names for Hakkar is The Faceless One, indicating a possibly connection to the Faceless Ones in Northrend, and YS, though to be honest I think this is more likely a slip of the writers, just like Hakkar was accidentally called the same name as a demon from a previous book.
