There look like there are 4 different offhand items, ranging from a bone with tiny dragon or animal skull and crystal, to a scarab, to a lantern, and an arm hanging on a wire like some grotesque lunchbox.

DPS caster offhand (the arm lunchbox?).
Healing offhand (the lantern?)
Feral offhand (the one with the animal skull?)

Maybe, just maybe, we finally get a feral offhand?
