You would have have no spirit regen going and a multiple of 5 in mp5 in order to get the perfect pattern going from BoW. Because mp5 is now on the 2 second tick with spirit. It is how much mp5 you have divided by 2.5. I have 92 mp5, so I get 36.8 mana from it on the 2 second tick. I also am currently running 257 spirit self buffed, which gives me 66.4 mana regen per tick while outside the 5 second rule. With 3 piece SR bonus + reflection, I am at 19.92 mana/tick from spirit. So as you can see, with all these things going, I won't get the perfect pattern from BoW. This is why you will see differences on this. But basically, the game keeps the floating point precision of your mana, but only reports the truncated whole number. If you have a mod that shows you your mana ticks as they happen, you will see that effect from spirit regen too.

Well, 1 in 4 people have uniform Spirit ticks (of Mages, at least).
1 in 5 people have uniform MP5 ticks.
Such "integral" individuals probably see a uniform rotation of 39's and 40's from Imp. BoW.
