I'm going to post this here because it is a bug and I've seen a lot of people who have had problems with it.

If you stand too close to the bushes and get targeted for a portal there's a good chance there will be no portal. You have to stand about 2 yards out from the bushes in order to not have this happen. If you've ever had problems with portals coming at long intervals, it is almost guaranteed to be due to people standing in the bushes.

Or it might be a Shaman that died, took his ankh and got portalled right away while his group is still in the realm, thus causing the intended next group to have to walk far or miss it.
