I guess all of you (or perhaps it's Alliance only?) know that the Onyxia buff bugs somewhat frequently and doesn't get cast when you turn in the quest. This happened to us on our first buffed Ragnaros try (though we pulled through more than fine there with our amount of buffs) and now again on our first buffed Loatheb try, with the result that we wiped on 7% when our healing chain broke when we actually had an excellent chance of having killed him at that point already, if the buff had worked.
After our incident at Ragnaros, more than a year ago, I had a talk with a GM who told me it was no bug but a very deliberate feature that it sometimes failed. He refused to explain how to avoid it though as they weren't allowed to give gameplay help.
To this day, I'm not sure if he was bullshitting me or not but, seeing as how many guilds consistently get the buff for Loatheb, I presume there is some trick to not making it fail. I'd like to get some information on how to avoid having it fail, and if anyone can actually tell me the purpose of it failing, I'm curious about that too.

On a handful of occasions, I've been standing next to someone who didn't get the buff while I got it, so there is some oddball chance of the buff not being applied. The same applies to the head of Rend-buff. I have no clue what the problem is though.
