When we were learning this fight, the healers would pull aggro on Roar after about the 4th fear (he'd come out of fear and come over to the priest, instead of going back to the tank). We solved this by having the tank stack 5 sunders, then fearing. He never came back after this.
If you have Salv, and you heal something in a raid setting, it distributes the highly reduced threat amongst all active mobs. This includes, AFAIK, trapped ones. It's a lot harder to see in such a setting, because the threat is small per heal.
I *have* grabbed aggro from a mob that was in a trap, such that it came out and came for me. Usually, yes, it goes for the hunter ... because any good hunter pulls the mob with a distracting shot and at least two auto shots, equiv to 3 or 4 sunders of threat before it hits the trap. It's hard to out-threat that with heals, but it IS doable. And I've done it. Usually it's near the end of the pull, where there's a mob left, and something will come out of a trap and rip me apart.
I'll get a hunter friend tomorrow and confirm this, but I'm almost positive trapped mobs accumulate threat. Since there are multiple mobs, and the threat is smallish (healing threat, that is), you may not notice it much, but as a healer, I've definitely seen something happen that can most easily be described as "in a trap, accumulate threat".
Ideal test is to get two mobs, pull them, sic the hunter pet on one, and trap the other, while the priest does nothing. Ideally, pull it into the trap with nothing more than body aggro. Spam a bunch of heals, and wait for the trap to wear off. If it goes for the hunter still, then no threat accumulated (since the hunter can generate 0 threat to a mob while it is in a trap, since all hunter threat generating moves are active -- pew-pew -- as far as I know). If it goes for the healer, then healing aggro accumulated. To make it interesting, make sure the healer does a lot of healing while it's trapped -- let the pet get beat on, and land a couple of 4k heals.

Rough Theorycraft on Roar:
- threat on Sunder x5 in defensive stance + paltry auto-attack threat
- threat generated per point of healing, with salv and all -threat reducers in talent trees, distributed amongst (say) all 5 characters in Wizard of Oz -- Tinman, Scarecrow, Dorothy, Toto, and Roar. Two of these are stunned or feared almost constantly, but let's assume for sake of argument that the threat goes to them as well, in equal proportion
Sunder = 301 threat. Assume Defensive Stance, and Defiance, so 437 threat per application. 2182 threat from sunders, call it another 700 from white damage done and random abilities used while landing those 5 sunders, so a nice round 3000 threat.
To produce 3000 threat on Roar, I would have to (with Silent Resolve, Subtlety Enchant, and BoS) do:
3000 / 0.8 / 0.98 / 0.7 / 0.5 * 5 = 55 000 healing.
It's a LOT of healing to out-aggro 3000 threat. A LOT. Assuming GH:7 spam, which hits for 5.1k, I'd have to heal 11 of those before I'd pull aggro. And I wouldn't do that in Oz. There's just not enough damage incoming to justify it.
But if you drop it down to nothing more than body aggro ... Roar comes out, runs at you, you sunder him once and hit fear ... that's maybe 600 threat. And that could be out-threated by 11k healing, which would happen fairly fast, assuming people made mistakes. Now, mostly in Phase 1 Oz, once you know the fight, there's almost no healing - a little bit as Tinman gets slowed, a little bit to top up Dorothy frostbolts. But very very little as compared to a boss fight. So most people would never notice.
