This happens quite frequently and I don't know how to reproduce it, but often times I dismount to tag something with an arcane shot or something and Omen sticks to Omen - GLOBAL and doesn't change to the target I'm attacking. If I change targets to something else, friendly neutral or hostile and change back to the target I originally attacked, I'll get the threat list for what I originally attacked.

EDIT: According to Party Revisions, they all have incompatible versions which is probably what's causing this.

This happens to me also, though I'm putting it down to incompatible versions too. Don't forget that if anyone in the raid is using Pitbull (with externals) and don't update regularly then the chance are they are using a very old version of Threat 1.0. On that note, I question the wisdom of including Threat 1.0 in the Pitbull package - it should really be an optional addon/module.

This is not while in a raid, this is while solo hunting on my alt hunter. I have threat loaded just for debug purposes and seeing a general estimate of my pet's threat on mobs.

Ah, right. This is likely due to the fact that Omen is off when you're mounted, since you don't have a pet. I'll see what I can do to fix this.

Just a heads up, had an issue last night on Prince M, my mage was registering threat through the roof. It _seemed_ to coincide with the proc of my Mystical Skyfire meta, but that's probably just me trying to attribute it to something. Once I would invis, it would work perfectly, but when I began the fight I was registering over 2k TPS with a scorch spam.
I'm sorry I couldn't give more worthwhile information on this one. It always happened around the beginning, phase 1, and invis would set it right and the issue wouldn't happen again after that. It registered raid-wide, so it was apparently reporting my threat at very high levels to everyone else.
Edit: Oh, r42522 of both Omen and the Threat lib.

During Phase 2 of the "Gurtogg Bloodboil" encounter in BT you gain the debuff "Insignifigance". while you have this debuff you produce no threat at all.
i hope this helps to create a bossmodule for this encounter...

There are couple of dummy modules in there for encounters not yet implemented, however I believe all of the modules for BC content are actual ones. They're also occasionally buggy, but they're still there.

Most of the old-world bosses aren't supported.
I don't know of any unsupported bosses in Outland that need boss modules, but I also don't know the fights in Hyjal and BT.

I just noticed in SSC today, that when I hit Invisibility, Omen reduces by my threat by 20% per tick. KTM does nothing unless it goes all the way through last I saw. I'm sold on Omen and will try to get my guild to switch as well.

The mod seems to really struggle with hydross at the momment, threat for nature seems to not reset, and threat for frost continues from the last frost phase or some such it seems, it's a bit weird, not quite sure what is happening

As Paladin I have my Judgement of command triggering -150 threat modifier instead of the -1000 modifier, even as the attack is using spell hit and is spell damage. Same with SoC procs, where the proc is spell damage but triggers the -150 threat modifier (As it should). Did I misunderstand your post, that it isnt showing correctly in Omen according to your description?

It appears that r42872 is not resetting threat on Leotheras at the threat wipes?
I updated to that version and personally didn't see anyone else's threat at all. I then reverted back to r42328 and was able to see the data from the rest of the the raid, but we had to manually reset the meter at every threat wipe because the people who had updated weren't going back to zero.

We did Leotheras yesterday, and I updated Omen just before that, so I think it was the same version (I'm at work right now, so I can't check it).
Threat was reset every time Leotheras entered the demon phase, but wasn't reset when he switched to human phase or after a WW ended.

Really all you need to do is get your people to get wowaceupdater and take 5 minutes to update it before the raid. Even while you are all zoned in ready to go take the break. There is also the nice feature that tells you if anyone has a newer version.
If you are using any ace add ons you should probably update them daily anyways, this will just give you incentive. It really is pretty simple.

I have WAU, but I cba running it every day just to get some yet another bugfix
I use it once a week maybe, but I can live without it easy, I got 3 addons using Ace - SCT, SCL, VanaKoS - none of them are required to update daily
If you are willing to beta test bugged addons - it's fine.. this is however doesn't mean all the other people in your guild has nothing else to do
People play a game to have fun
I want to install an addon and forget about it for a few days atleast, preferably a week+
I want it to be tested before it gets uploaded so that I don't have new "undocumented features" every time I log in
I don't want to get in a raid and find out that my version is borked, because someone just broke a protocol with a new change and I can't see anyone on a list now since then I need to spend 5-10 minutes of raid time on updating addons while we should be killing bosses instead
(Therefore I use KTM)

It takes literally 5-10 seconds before every raid for me, so I have no problems updating all the time. But as the author himself has repeatedly said, Omen/Threat is in BETA, so use it at your own risk.

Surely neither of those is correct? Doesn't invisibility tick something like 10%, 10%, 10%, 10%, 60% over the course of 5 seconds, for a total of 100%?

This is in, as mentioned, but this is not in:
One people gets a debuff called "Fel Rage" and he/she is the target of Bloodboil at phase2. The one who has fel rage gains extra damage and healing powers and does not produce threat as well, because after fel rage ends Bloodboil goes for the main tanks even the fel raged person exceeds their threat level.

T(1) = 0.8*K
T(2) = 0.8*T(1) = 0.64*K
T(3) = 0.8*T(2) = 0.512*K
T(4) = 0.8*T(3) = 0.4096*K
T(5) = 0
That's my understanding for how invisibility works.

Does Threat include the Feral Druid 2t6 bonus for 15% more threat from mangle? I've just looked through both KTM and Threat (slow day at work) and KTM definitely doesn't have it as it spells out each and every set bonus that it includes, while all I can find in Threat is a generic string to check for if there are any threat modifiers from set bonuses. Will this work for it?
