Antiarc, there seems to be an issue where some threat is still coming through during "Insignifigance" in the Gurtogg Bloodboil encounter.
The below screenshot was taken from the most recent build available on the 10th August. (my positioning of the bars tend to hide the version at the bottom, oops!)

As for the rest of the raid that gets insignificance, the Threat module is working in applying no threat given throughout phase 2.
However, for players who get Fel Rage, I don't think [I may be wrong] Antiarc has not implemented that yet and therefore people with Fel Rage appear to be above the main tank because of all the damage that they do in that phase.
I hope that makes sense and helps.

Well, I was more concerned that 5 players listed on Omen (myself included) during the fel-rage seemed to have a constant trickle of TPS continuing when it should be 0. Stigmata who was tanking Gurtogg in the screenshot is listed as 0 TPS and not above the tanks, so that would indicate that Omen is working OK with the fel-rage target warpandas.

In the 66 pages of this thread, this may have been covered already and I missed it. Apologies if so.
Last night I had my entire raid install Omen. I was suddenly unable to open my raid frame to change group members while in combat. The only changes I had made were to enable Omen and disable KTM. I had been able to change group members no problem earlier that night, and other members of the raid with Omen were able to move groups fine as well.
Thoughts?

That's odd... Delete all of the addons that begin with Blizzard_ in your addons folder (they'll be recreated when you login next, maybe something got corrupted).
Not sure what else to suggest, I don't think it's anything of Omen's doing

This one's been up in the thread before, see posts starting hereish.
Short answer is that power gains are usually exempt from threat multipliers and threatlib assumes this is true for Bloodboil as well, which means that Vampiric Touch, Master of Elements, Innervate, Evocation, Unbridled Wrath and the like are generating threat on Omen for their relevant classes when under the debuff. Which explains the various trickles and you, no clue what's up with Dukes the druid there - is it possible he resisted Insignificance?

Something was going wrong on Mag last night, a mage who was significantly below me pulled threat. Saw this multiple times actually (had click issues =( ). I do not know what the issue is I just know that it happened more than once to the same mage and also happened to a rogue.

Also of note is the time that it looked like the mage pulled threat... but it just ran after her, then turned around and came right back to you.
Something screwy was definitely going on.

Did the mage invis?
There is an issue on mag where dots can sometimes prevent threat from dropping on him (rogues have a hell of a time with it).

We did Hydross again the other night, and again had the issue with our Frost tank vanishing from the threatlist. He goes up normally, then his TPS tapers off until he hits zero, sometimes he only is there for a few seconds (just long enough to get the misdirect threat.) We have had this issue with him on Prince in Kara as well. I had him tell me his mods, and didn't see anything out of place (he had many of the same ones I did.) This is seriously getting to my raiders, and they are starting to lose faith in the mod when one of our tanks is consistently not on it. Is there anything I can get to you information wise to try and isolate this problem?

Noticed an issue today. I logged in while in combat (lagged out), and I could see a few members of our guild. They all run Omen, and were on the same version. Some however I could only see with KTM data. (They were still publishing)
It was very, very strange. Once I reloaded outside of combat, everything went back to normal.
Just thought I would mention this.

I have posted something like this on the wowace forum:
The first thing i want to say, realy good job with this project !!!
I followed this project from the start and it only has gotten better and better Cheesy
I only got one little weird thing ... since i convinced my guild to change from KTM too Omen.
When i display 'Show party revisions' some people don't show up as compatible.
Then when i reload my gui, i do it again and totally other people don't show as compatible.
I noticed yesterday too that one person who shows as compatible, had an asterisk behind there name.
So there seems to be a bug in this, although those people are running one of the later versions.
Is there maybe a check only at the beginning of joining a raid or how does this work?

Antiarc said it would be removed as soon as the Mod gets out of Beta. Currently it is there so people know what revisions they are using if they do find a potential bug.

I posted this on page 43, so its easy to understand it got lost in the shuffle.
You can modify the lua to remove it. It was bothering me too, so I quoted out that line in the lua. Look for a line that says:
-- App revision string
Just below that, there is a line that says:
self.Anchor.revision.SetText(string.format(.........etc..
Comment out that line by putting " -- " in front of it. You have to modify the file using Notepad, as using Wordpad will change the text formating and cause Omen not to load correctly.

I often read this thread and i know that somebody wrote about an addon for multi target with the same name, that can handle with omen.
But i don't find this addon.
Can you help me pls?

It's called Omen. Multi-target is in there by default. If you want multiple displays, look at any of the addons that use ThreatLib data; Omen, Assessment, Recount, Aloft, Pitbull, etc. One of them, possibly more, will probably suit what you need.

It's uqite possible as the AoE-resistance component of Predatory Instincts seems to work on Insignificance. I've had that happen a few times today while training Gurtogg and it's pretty nifty as a tank, I might add.
We didn't have any issues with threat-gains despite Insignificance today, but Gurtogg's knockback ability ("Eject") seemed to throw off our Omen by a lot whenever it happened. I might add that we used three tanks on Gurtogg which made inaccuracies a lot more noticable, especially since all three of us tried to ride a fine line together without leaving the warrior behind.

I'm checking the Bloodboil module and it calculates Eject at a 25% reduction right now. Does that seem like too much or too little?
I think the script command for temporarily changing the Eject threat reduction used by your raid is /script AceLibrary("Threat-1.0"):SetNPCModuleValue("EjectMultiplier", 0.8) for using a 20% reduction instead, for example. You could try playing around with that; setting the 0.8 multiplier to other values and seeing what's accurate. Also, hopefully Antiarc will be along to correct me if I'm lying about how this works. :p

[Prism of Inner Calm] is valued too high I think.
I pulled aggro at 100% and had lots of crits.
Edit:
Found it.
if trinket1ID == 30621 or trinket2ID == 30621 then

I've been following this thread, and the addon since this threads creation, and often saw reports of a tanks threat halting/not updating for very long after the pull. However I did not see any solution to this problem. My guild made the switch to Omen around a month ago, and since the switch we've had issues with one tank not updating, and intermittent issues with others depending on the release.
With the most recent versions of threat and omen installed (46712 and 45044 respectively) for both myself and the tank in question, his threat 'halts' in raid settings, but in 5 mans the issue does not present itself. The threat values apparently work fine for himself, and we've resorted to having him call out on vent when someone's getting close to pulling. Needless to say this isn't an optimal arrangement. Also of note is that some people are saying they can see him.
For those who have also had the issue what steps did you take to correct it? We've tried fresh installs and removal of associated savedvariable files and haven't quite found a fix to the issue.
Any help or suggestions that could be offered would be greatly appreciated. If needed I can fraps a bit of an upcoming raid as well as a screenshot of raid versions if it would help.
