I have to second this. For a long time Utopia was broken, and before that it was just kinda meh. I'm not sure if somebody new picked it up or if the author just took a renewed interest, but in the last few weeks it's become a very robust buff/debuff tracking mod.

The "portrait" appears to be ZMob DB, though the unit frame could be any number of addons.

Related to the quoted full screenshot, does anyone know what addon was used to change the minibuttons to HoMM5 graphics?

There are doubtlessly a few mods that do this but I use Big Brother for such purposes on raids. It gives you a line of the form:
"[%t]'s [Raid Symbol] [CC type] has been removed by [Playername]'s [Action-name]."
