The other part of this plan I'm not sure will work well is the issue of how much screen space Grid takes up for each size group; my monitor is annoyingly small and so I tend to have everything squished together pretty tightly. I could see that being fine for 25 people or less, but what about 40? It's unlikely I'd just be able to stick the extra 15 bars on the side somewhere, since it would almost certainly start overlapping other aspects of my UI, but I'd love to have my UI change as little as possible as I move between raids/arenas/BGs/other settings.
Any ideas or advice would be welcome (as long as it's more than "go buy a bigger monitor"!).

I use grid as my only UI, with gridstatuslifebloom of course. I don't think you'll have a problem getting rid of other UI's and effectively healing. Grid will definitely take care of any problems you have, you might need to get a grid mod or two though. I used to have the same problem with switching UI layouts from tanking to healing. I put some time into getting my grid set up just right, and now it's the only thing I need.
Also, I assume you are hiding party portraits while in a raid group. If screen space is an issue, not doing this will kill you. You can always change the size of the grid if your monitor is THAT small.
