1) Yes, Firestone for Demonology and Destruction. Spellstone for Affliction.
2) Whenever you feel its useful, or in some cases, whenever the raid lets you cast from melee. Most high end raids require ranged to be at ranged, whether it be because of adds or because of debuffs. Meta is a fairly quick cooldown and can be used on trash as long as you make sure its going to be up by the next boss fight.
