I believe it's stated in many places to read through the entire thread before posting a reply.
On a less beaten-to-death issue, I just spent a little under 2 hours casting outbreak at a raider's dummy while just over the spell hit cap, and in 112 casts had 0 misses. Hardly conclusive, I realize, but unfortunately I didn't have time for any more.
