My (way to few) samples would imply 1) extra attacks are treated like yellow attacks (no glancing, no hit penalty, maybe even non dodge-/parry-/blockable), 2) offhand and special attacks work alike, the timer isn't reset for neither, 3) rather not.
As long as 2) holds (extra attacks triggered off offhand attacks do not reset the swing timer), Sword Specialization is superior to Fist Specialization for any SS-oriented weapon/talent setup, as you'll typcially end up with a 25%/50%/25% pattern for MH/OH/special triggered extra attacks: for MH is adding more damage than a crit (poison, and crit chance for the extra swing), for OH it's adding a lot more damage (poison, crit chance, and MH:OH speed), and only for special attacks it's yielding less (about 70-80% for SS).

1) I don't know if the numbers are actually yellow, but the attacks cannot glance. No off hand penatly. MH Weapon damage + AP / 14 * Weapon Speed
2) No, the swing timer isn't influenced by sword spec procs
3) I'd go with no, but as for as I know nobody has done any extensive testing yet
If your choice of weapons is sensible (a slow main and a quick off hand), sword spec will out-DPS fist spec.
