1. Except for the alysrazor encounter where you need 24 expertise but still no hit, reforge everything into mastery.
You can try to even the difference between your dodge and parry rating when you hit a mastery value of about 24.
At 24 mastery I feel that the loss of survivabilty in uneven ratings of dodge and parry (difference of 500+ rating points) is greater than the benefit of reforging more into mastery.
You dont't need expertise or hit because from what I have experienced with a low expertise / hit value is that your DS are still hitting most of the time (which means that they are missing for me at 5-10% of all DS). The survivabilty gain of mastery vs. expertise definately favors mastery in a huge factor.
Things stated above rely on my personal feelings about the damage intake in the FL. I currently have no proofs but I'm about to analyze my logs to ensure I'm not totally wrong.
2. I would reccoment using the Scales of Life + Mastery trinket like the spidersilk spindle or if you don't have it the TB trinket as it is still a huge cooldown to have on a 2 min base (see riggs post).
