You can tank T5 level with that gear with ease since I did it with roughly equal gear before I dropped my Druid ( The World of Warcraft Armory Imagine a Wildfury in the MH).
If you want to improve the gear you already have, you need to keep farming Scryer rep for the better shoulder enchant and use a [Nethercleft Leg Armor] on your T4 pants if you intend to keep it for a while. Replace your chest and head gems* with [Solid Star of Elune] and get [Band of the Swift Paw] since you can easily afford the loss in resilience.
*if you want to keep the socket bonus (I did) you can instead use an [Enduring Talasite] on your helm.
Logical gear upgrade path is T5 raids and ZA for you.

thing is, since 2.4 will be launched 'supposedly soon', i am contemplating whether to wait till then or change now. lol
