Personally I'm PvPing as combat at the moment (and PvE'ing as Mutilate, how backwards :P ) so Shiv is a massive part of my playstyle and it needs spamming somewhat, it's an unavoidable combopoint for a start when toe-to-toe'ing with other melee which leads up nicely to an undodgable finisher 
For the majority though mutilate spamming their way through the rankings - i'm sure shiv isn't even on their action bars
