I am not familiar enough with wowmeter to get right into the details but my guess would be a combination of luck and a missing melee buf. One possibility is the doomguard missout on heroism due to being under some obstruction when it was activated but since it should have been attacking patchwerk when heroism was hit that seems unusual.
