For starters I don't think the WWS you provided is really going to show anything (I TAKE THAT BACK, SEE BOLD BELOW). You only MT'd Thaddius, the rest of the time you were either subbed or the 4th tank in the pecking order. A warrior tank requires rage, you can not be expected to push out maximum threat when you have zero coming in and changing your spec around will do little to change this. MT Patchwerk and put up the WWS from that, it will give a much better indicator of how much threat you can push out.

Some quick things I noticed, you need to spec into deep wounds as it will add around 10% extra damage for starters. I don't like macros for shield slam/revenge. What happens when shield slam is almost coming off CD and the macro decides its best to revenge, although revenge is the best rage per threat ability its still second to shield slam. If your putting devastate as a very low priority then that can also be very bad, the main reason is that revenge/devastate will trigger sword and board which increases your shield slam/devastate crit chance by 15%.

I just noticed, "YOU WERE MT ON THADDIUS AND YOU ONLY DID 6 SHIELD SLAMS and 6 REVENGES" !!!!!. Unless its a glitch with WWS that's a really glaring stat. On Thaddius due to its gimmick mechanics your shield slam can hit for 14k+ and I think your highest was around 9k. You used HS 148 times which is good however your #1 GCD ability was devastate and its biggest hit was around 4k.

I noticed in the WWS your heroic strike did more damage than any other ability (NOW I KNOW WHY, BOLD ABOVE), if your the MT on most fights through the night then it might get close, however shield slam should still be #1 unless its a rage infinite encounter on a single target. Considering your #4 in the tank pecking order heroic strike should be really low as rage will not be at a premium. You did tank Thaddius and the rest of the night it looks like you were taking a break so this could skew the stats. Even on trash your heroic strike is really high, on trash cleave takes priority over heroic strike and your shield slam is like #6. Shield slam although not as useful on trash you need to have 1 mob at least hitting you to feed you rage and shield slam is a way to keep it on you so all your other abilities can be used as soon as they come off CD.

Is there a chance your macro for SS/REV bugged out and although you thought you were SS/REV you actually were not?

My suggestion would be to scrap your SS/REV macro and use the priorities below, although not perfect for every situation I am sure you would see a big improvement.

Single Target : CB+SS -> SS -> REV -> SW -> HT -> DEV (HS when over ~50 rage)

Trash/AoE : CB+SS / SS (Main Target) -> SW -> TC -> DEV (CLEAVE when over ~50 rage)

Remember Shockwave's effectiveness will change based on when you do it, I tend to shockwave when I have maximum rage so I can dump out more threat while they are stunned. Stunned mobs with zero rage = zero rage and no threat.

Shield Slam has 700 innate threat from the top of my head on top of the damage, more than any other ability by far its #1. Shield Block is not required as a survival ability most of the time and considering it doubles your shield slam damage I'd say its pretty good for burst threat especially at the start of encounters (It also helps you take on more mobs as the damage for the next 10 secs will be much lower while its active). It should be used whenever off CD and also can be timed with other CD's to maximise threat as well.

Also get your self a timer mod for your abilities, I think one is called CooldownCount. If your in GCD and SS is less than 2 secs away, don't use another GCD ability and wait that split second for SS.
