I think it is incinerate and conflag, with immolate being replaced by LT glyph without soul leech.
Re: 'rotation' - I have slightly higher lag than others and find that doing immo, CB, conflag, incin spam works best for me. I tend to have what I perceive to be too long between casting immolate and when I can cast conflag so I throw CB in there. The timing works out well in that CB is off cooldown about the time I need to refresh immolate.
