yes!you don't need to keep your disease on your target all the time,it will fades out for 5s in each 20s,but never mind!
just make sure you can cast 4 OB in the diseased 15 seconds,and then,Frost Strike will also get buffed by OTHER DK's disease!
5s after your disease fades out,you can use PS and IT to attach them again,and cast 4 OB...and so on...
I think it's a good news : )

Need more details. It's hard to see how 15/56/0 would be better. Any proof of these two claims?
