I see alot of similar setups to mine; unitframes bottom middle, disease/runetimers above that, buttons below that.
This is working great for me, here's a screenshot:
Green skull means Death Trance proc, blue halfmoons means I don't have Horn or Str totem. Next to the cooldownbar is usually a petbar for extra symmetry, and the empty space between the map and the buttons is where omen goes.
One addon I haven't seen many people use is RunePack which has been discussed at length here: Death Knight rune addons: How can we do better?

Few people messaged me about my UI so just to clarify here, it's based on Raynes Burnt Cherry available at wowinterface.
A little further explaination of how I use Runewatch, Power Auras and Elk Buff Bars to show the info I rely on to dps.
Please dont ask me how to do it, its far too much to explain but easy if you play around for a while.
Runewatch is the circular thing with my runes around it, it can track how many of your diseases are up on target and show you the countdown for each. Above it is my timer for my unholy blight so I know when to refresh it.
Other things I've added to it with power auras.
Presence Detection:
Blood Presence : Frost Presence : Unholy Presence

If Unholy Blight isnt up at all I get a very large warning...

No unholy Blight up
----------
It also warns me if Horn of Winter or Strength of Earth arent up (they dont stack).

No How or SoE up
----------
If my Bone Shield drops I get a warning for that also.

If no buffs are up at all it looks like this

What I like most is that it's all in the place where I look almost 100% of the time... my characters feet to check if im stood in anything nasty :P

Quick question about Power Auras.
Is it possible set complex conditions? For example:
if Unholy rune AND Frost rune (death rune can be counted as unholy and frost) will not be available in 1 sec then: show some icon.

I have update my UI, cleaned it up and streamlined it a bit, and here is the result:
Most of the same addons, with the addition of SLDT (the text next to the minimap), and KgPanels for aesthetics. I can find no better way to show my runes and diseases as of yet.

What are the disease timers being show in bar format immediately above your unit frames? Are those satrina aswell?

Unit Frames show only the important things, near the center so I constantly see them. I only track Frost Fever and Blood Plague as you can see.
Wrote some dogtags which indicate Aggro very easy.
Killing Machine proc is close to my UF so I immediatly see it.
AtrRunes .. numbers, easy to see .. close to my UF.
Debuffs are quite huge, cant miss em
Replaced all possible SCT mods and using EavesDrop (the thing in the right corner), this keeps my screen uber clean even when fighting over 9000 mobs.
Important skills on clear actionbars so you can see the CD quickly.
Party Frames go in the middle center, around the height off the decease tracker. They only show MissingHP in either Cyan colour (no aggro) or in Red (Aggro!). Mouseover function to see maxHP, if they are at MaxHP.
The UI is made to be clean, very informative in a very easy way. Important things are seen the quickest. And all in all I couldnt bear to make an ugly UI so I spend loads of time making it pretty
Oh and, Omen goes above the actionbars. Hope that's enough information.
Comments welcome.

Probably just Bartender or Dominos skinned with Buttonfacade.

Yes. If you go into the Satrina options, then Units, you can select a frame for your targets debuff frame. I don't like to see Crypt Fever, and it doesn't show up under the normal Spells options (nor do any other debuffs it seems) to disable. It seemed to work fine just Shift-Clicking on the icon and choosing to not display it though.

Apparently with all the goofing around with the howling blast cooldown, they've manage to remove the display for the cooldown altogether. I originally kept a CDT3 cooldown bar specifically for it right next to my runes, but since the game isn't reporting a cooldown for it, CDT isn't picking it up.
Is there any cooldown timers that allow you to manually add a custom cooldown timer any more?

It's a bug in the blizzard interface. I'm not a programming expert by any means, but I don't think any cooldown mod will display it right now. Unless you macro HB into starting a timer mod for 5 seconds or something.

I'm sorry if this has been stated already on this thread, but I couldn't find it, but I was wondering what add on could be used to display (preferably VERY blatantly) when I have a Rime proc? I just specced frost after patch day and am having trouble spotting it amongst the plethora of buffs I usually have.

PowerAuras should be able to do this for you. I haven't tested it specifically with rime, but used it for similar things on my shaman. It takes a bit of configuration, but works very well. You want the new "PowerAurasClassic" version, which has been ported to LK.

Reorganized info a bit to a right weighted feel, just didn'd like having chat/recount on the left. Not releasing this one publically. Addons are same as Sympa UIv4 only switched to pitbull over AGuf for Dog Tags. Fonts used are Helvetica in two different weights for Pitbull and chat/recount. Submitting as a video so folks can actually see the function of the UI.

Since the release of the expansion, I've tried to keep my interface clear and simple. I've cut back on mods and what not since I needed a huge amount of add-on memory consumption for Quest Helper (the thing takes up more than my entire UI).
However, since my rogue was fully geared and my death knight hit 80, I realized that since mobs weren't dying before I could even use all my runes, I needed a better way to monitor them than the default UI.
After some trial and error I've come up with this:

I tried using MSBT at first, but I didn't like the way it scrolled, and I was rather fond of the game damage. I may decide to go back to it later, but for now I'll stick with default+the original SCT.
Pitbull for the unit frames, and Magic Runes for the rune detection (colored bars above my player frame). Between the target frame and the bars is where the target of target frame appears.
The UI is set up to provide me with all the information I need in places that are very easy to glance at without having to move my eyes to far. I'll watch my character to check my spacing and watch for AOEs, and glancing to the left I can keep an eye on my rune timers, runic power, and relevant DPS cooldowns. Checking where I'm standing I can see if I'm standing in anything dangerous and also see my swing timer, global cooldown bar, and disease timers.
The rest of the UI is designed to be aesthetically pleasing, but as I'm no good with eePanels or other similar mods, and I refuse to download compilations, this is the bet I can do.
The bar near my player frame has any cooldowns I may use in pvp or pve, but none of those are bound. All of my abilities are bound and hidden. On the bottom I have things I may click like my vent-macro button, army of the dead, vanity pets, mounts, and professions. I think I may set them to fade out during combat, but who knows.
Oh and then I use Power Auras for lots of things: gargoyle, icebound fortitude, lichborne, unbreakable armor, unholy blight, killing machine, freezing fog, and my stances.
The green artsy-looking circle on the bottom changes colors depending on what my stance is.

This is the current UI for my DK, currently at lvl 77 so no doubt it will change in a few levels once I get to 80 but not that much!
Edit:
Power Auras showing
Skull - Killing Machine Procced
Bull - Horn of Winter NOT buffed
Little red stroke near rune frame - Rune Strike available.
Screen shot was taken when server died and I was stuck not being attacked hence why Buffs are dead and i'm not where I should be

Well here's mine. Had to use a combat screenie to get all my addons in action.

A little bit of info regarding the setup, I've decide (unlike alot of people) to attempt to space everything out rather then have it in the centre which usually results in tunnel vision [Which resulted in me getting smashed on 3D OS]. It's built more like a 'frame' so I can see what is actually is happening.
Though I do feel it is imperfect in the sense that it feels like there is too much of something, maybe its colour, or maybe its boxes lol.
