Can we please stop with just random shots of various UIs, a spot of mod listing, and actually discuss the theory behind creating an efficient Death Knight UI? There is already a thread for posting your UI elsewhere in these forums, which I enjoy looking at for inspiration, so duplicating it here seems a touch unnecessary.
I gave Sympa's UI a trial run last night for some heroics, and whilst it shows sound design, it didn't quite gel. I found that I didn't really need the additional magic runes display of cooldowns and runic power, as the ag_uf rune display worked perfectly well for that. So, I chopped and changed and substituted a few things (Need to Know for Class Timers, ditching a lot of the unnecessary text from the unit frames), and I'm much happier with it. I do think that the Sympa theory is fairly sound, but personal preference will override even the strongest design.

Well, as far as efficient UI design goes, one of the big things that I've tried to do is use the infinite height/width of screen edges and corners to place my "clicky" action bars. That way, in the heat of the moment, you don't need to be careful with where you click. I've also been using AutoBar to help auto-manage the crufty items I seem to always place on actionbars like quest items, potions, bandages, etc.
For Unit frames and the like, I still haven't found a good location for everything. Of course, you want to try and keep everything as close together as possible, but what's a good, efficient way of doing that in a way that doesn't look horrible?

Here's the screen, basically I use ag_UF's own rune timers to keep track of them, has worked for me so far. Besides, when you get your rotation going, you don't need to see the runes at all, just the CD of Obliterate.

For runes I use Xperl and OmniCC, it shows the Cooldown on each rune.
It has been enough for me, I only have to watch the top left of my screen to keep track of my runes/RP/Disease & HP.

This is the UI I made. The core add-ons are Pitbull, Runewatch, MSBT, and Bartender. Omen is normally in the bottom right. I focus on simplicity while keeping most of the relevant information around my focus point, i.e. my character. There's nothing more annoying than looking off-center for something or trying to see through clutter during a fight like 3 Drake Sartharion. That being said, I do need a better way to keep track of proc buffs like Greatness. At the moment, I rely on MSBT announcements and counting in my head. People have recommended TellMeWhen or NeedtoKnow, so I'll be giving those a shot.
The only issue I've encountered so far has been the rare bug of Runewatch where it says I have runes up but I'm unable to use them. Other than that I've been pleased with its performance.

Don't know if this has been covered elsewhere, if so i apologize beforehand.
I'm having huge problems with Bartender 4 and skill CD's. It's completly inconsistent with runes, specially juggling Death runes. I'm currently Unholy spec and one problem which often arises is that the visible CD on my SS button never takes into account death runes.
2xBS followed by 2xSS leaves me waiting for 2 death runes before the next SS. The cooldown on the button however ignores the death runes and goes by when the next Frost + Unholy rune becomes active. As soon as the death runes pop, the button becomes colored again and i can do a SS, BUT the button itself still got the shaded clock-timer ticking down til the next U+F rune.
Am i making sense here?
This has had the UPSIDE of me learning to not trust the buttons, but rather focus on my runewatch, so i guess in the end i'm better of for it, but it's still a tad annoying and i'd like to know if anyone else experienced it.
Also Bartender 4 seems to mess up my CD on Bone Shield quite alot. It shows it as a colored button with no shaded CD, but when i mouseover it the skill still has 40+ seconds left or whatever. I assume this goes to the CD sometimes beeing calculated of the Unholy rune i used to pop Bone shield.
Is this a problem unique for Bartender or is the fault somewhere else?

The problem exists in my Bartender as well, so don't think it's specific to you. Haven't found a fix for it yet, the only advise I can give you is use a good Rune tracking mod, and watch those (like you said). I find it working way better because you constantly know the state of your other runes as well.
The bone armor thing is really annoying indeed, it seems it does not use the skill cooldown unless you have runes up to use it and spam it a few times.

My current UI.
The rune/disease tracker mod is Runewatch, around it I use power auras to show what buffs are active or not.
For example when I need to get UB up there will be a glowing green edge around it.
If HoW isnt up there is a blue pulsing ring to signify that.
I'll update with shots of the runes when I get time.

I have been thinking of a mod that I feel would be very useful, inspired by ShockAndAwe (former DisqoDice) for enhancement shamans. Basically, have rune cooldown bars similar to Magic Runes and on top of those have our disease/unholy blight duration. Also have all the bars go the same way, and have them to be different lenght depending on the total duration of the disease/unholy blight/rune (say 10 cm for a rune cooldown, would mean a 12 cm bar for am untalented disease duration). Would be very easy to see when to refresh diseases then, and would save a little bit of brain power to be used elsewhere.

I use a setup very similar to Sympa's, although I can't post pics from work.
I have ICE Hud set up with just my health and RP in vertical bars, with a sortof panel to the left of my health containing, from top to bottom: DoTimers limited to only my diseases and DnD, MagicRunes horizontally laid out, with no RP bar, and TellMeWhen set up to monitor when I need to reapply debuffs, when CDs are available, and when buffs proc (for timing the Garg).
This really does help me to prioritize, as a glance at my runes in the center can tell me if my diseases are running low, if I need to reapply any buffs or if I should pop my Garg, and how much damage I am taking. I have plenty of time to watch the rest of the environment, and so far I haven't stood in the fire due to trying to track all my timers.
