I just got Twisted Visage and I have Origin of Nightmares with Mongoose on it. I'm debating putting Blood Draining on the TV for some raid testing, but everywhere I read says it's not better. My question is, does anyone know for sure if the stacks can proc AND heal WHILE you are under 35%? Or do you have to be above it and then fall under to stack and then trigger?

I am in the same situation as you. And I can confirm it. Yes when you are under 35% a proc will heal you instantly.

However I wonder about why you would put blooddraining on TV instead of OoN? TV is the better weapon but it has less stamina. It is the ultimate non-stamina depending fight weapon there is. If you want stamina, OoN is your choice. Now when would you want stamina? In fights where you will need it! So in those fights you will often get hard hits and drop below 35%. In my opinion blooddraiing is the best choice for OoN when you got TV (on which you enchant mongoose).

This coincidentally makes TV even better for dps as well.

That assumed that blood draining was at all good. So far as I've found, I'd rather have the miniscule extra armor from mongoose over the heal. Especially since the proc system it uses is the same fun one that imp lotp and SD both use, meaning server latency can cause blood draining to overheal (something that in theory should never actually happen).

Basically, on fights where I can reliably be dropped below 35%, I'm finding that it triggers too often to make a noticeable difference in health. When I'm not reliably being dropped below 35% I've found that it's simply useless.

You mean it doesn't proc often enough? Or how should I interpret your post?

If I'd drop below 35% often in a fight, more often than not I'm doing so quite often (Ignis the first time I tanked him is a good example of this). In that fight I could be down there quite often - 4-6 times a minute.

In that situation, you'll get a heal of 400, since it takes 10 seconds per proc and will be used immediately when you hit 35%. Which makes it functionally equivalent to about 35 stamina, except it's stamina you can't rely on.

The ideal case is when you will drop below 35% once in 50 seconds. In that specific case, it'll act like a 200 stamina boost right then. There are fights that have attack characteristics like this (Mimiron P1, Vezax, and possibly Hodir) but most fights are much more dynamic.
