It regems your entire gear set, not just the item replaced.

Regarding your Kill Shot Low HP gain bug, I'm not sure I understand. On my sheet these values are always 0 when using the rotation test. This section is only used by the frequency calculations.. and then only if Kill Shot isn't in the shot priority list. It simulates adding Kill Shot to your rotation when the boss hits 20%, while if you have Kill Shot in the rotation then it assumes you can use it 100% of the time.

Maybe an explanation is needed about the Gear Planner.
It works likes follows. For each item checked it does the following:
Pre-loop:
1) It saves your currently equipped set in memory
2) It determines a baseline DPS value. This is your dps without any item in that slot (also without enchants gems for that slot).
3) It restores your current set from memory
Loop:
1a) It equips an item.
1b) If the item was your original item, it uses the difference between your current TOTAL dps and the saved baseline value. It saves this and skips steps 2 and 3.
1c) If the item was the special 'Current item with Regem' it regems your entire set and saves the difference between the baseline DPS value and the new total DPS value. It then skips steps 2 and 3
2) Depending on your setting it fills it with the default gems for that socket OR it regems your entire set completely to (hopefully) optimum values.
3) It determines the difference between the baseline DPS and the DPS with this item and new gemming and saves this.
4) It restores your original set saved before the loop from memory.
The Gear Planner thus shows the following:
The yellow 'current item' line shows the dps your currently equipped item was worth compared to NOTHING in the slot.
The delta values above it are relative to that value, which is obvious by the fact that your current item is outlined in RED and at 0.00 delta.
The green 'Current Item with Regem' line is what your current item would be worth if you'd regem. This ONLY should show up if this is an actual DPS increase. If it does then that is your 'new' 0-point. Any item below it is a dps increase purely over your original 'sub-optimal gemmed' set, but a dps decrease compared to your current item WITH a regem. Any item above it is a dps increase with a regem.
So the Gear Planner shows you 2 things:
1) Whether you can gain dps purely by regemming
2) Which items are a dps upgrade over that even after regemming.
You should always press the 'regem' button after you equip an item the gear planner says is a dps upgrade, even if the green 'current item with regem' line is absent. If this actually leads to a dps decrease, report it here or PM because it means the regemmer is messing up.
Still from my own testing the regemmer isn't always 100% optimal, but it's within 1-2 dps of it.
