If you're using Omen, what you're experiencing is probably related to having the list too small. The list shows you in the third (or whatever you show as last) position with the tanks in the first and second, but it's really omitting everybody between you and tanks because the Omen interface is prioritising the display of your threat relative to the tanks rather than including everybody above you. In other words, make the window bigger in order to see everybody that is above you in threat.

Are you tricks trading with another rogue? When 30 seconds expire, the threat will drop off. This would be about when the other rogue would tricks you again, once more raising your threat, but there would probably be enough time for Chimaeron to knock off a few people first.

I don't think Omen is 100% accurate. Neither is recount or any other 3rd party addon like that.

Would you be running out to try to kite it first? That gives you an additional 20% threat buffer running at range.

Phrequency gains Phrequency's Cloak of Shadows.
Ignition's Ignition hits Phrequency for 25312 Fire.
Ignition's Ignition hits Phrequency for 28262 Fire.
Phrequency's Cloak of Shadows fades from Phrequency.

As far as I know Omen pulls its values directly from the WoW API, and there isn't anything available that is more accurate than the WoW API.
The times where Omen calculated the threat values by itself are long gone.

However according to its description on WoWAce, it still estimates the temporary threat gain for TotT, Misdirection et al.

I've always assumed this to be true because of the behavior of the threat bars during TotT. The threat bar doesn't continually grow, it just has another bar added on to it, so I always assumed the additional threat wasn't "official" in value.

Should be worth mentioning that Omen is limited to the update frequency of the WoW API, which is very low.
At least the authors of omen complained about that when leaving their own threat calculations.

Without timestamps, but one ignition:

x gains Nethiri's Cloak of Shadows.
Ignition Ignition failed. x was immune.
Ignition's Ignition hits x for 36761 Fire.(11488 Resisted)
While the immune part will always display only once in your combat log, you can get up to 4-5 immune ticks if you time it right. Just try it out yourself when a wipe is called.

From the 4.0.6 patch notes:
Cloak of Shadows now functions properly to protect the rogue from the following boss and creature abilities: Gravity Well (Terrastra, The Bastion of Twilight), Burning Corruption (Crimsonborne Seer, Grim Batol), Lava Fissure (Slabhide, The Stonecore), Geyser (Lady Naz'jar, Throne of the Tides), Noxious Mire (Naz'jar Sentinel, Throne of the Tides).

Interesting that they clarify what abilities can be cloaked in Cataclysm Heroics, yet we still scratch our heads as to whether or not cloaking Ala'kir's Wind Burst or Nefarion's Electrocute at the last second is intended or not.

This confuses me, what is cloak "protecting" us from for Gravity Well. Does cloak prevent you from being sucked in, from the well spawning, drops the grounded debuff correctly as it currently does not?

For the most part, this seems rather bad. Changing our survival spell to make it more likely to kill us seems rather ridiculous. Either we can't be sucked in and therefore can't get the debuff, prevent the well from spawning and anger your raid, or drop the debuff when we cloak lightning rod. None of these seem to be a pleasant area, especially since cloak's standard effect does not stop Thundershock.

If the idea is to dissuade us from cloaking the lightning rod, why not make it so lightning rod can't be cloaked off?

Edit: Separate topic: Would anyone second the idea of stating that mutilate should be recommended for the Twilight Council? The elemental Monstrosity is definitely the push phase, and it occurs starting at about 30-35%, for Assassination's execute range. Combat has a bit going for it with potential for cleaving above 35%, but I don't see a lot of strategies bringing the bosses together. And by not a lot, I mean I haven't seen a single one yet.
