In looking through other class forums I've noticed a number of them have UI discussion threads. There are references to UI and addons through some of the threads in this forum but no single place for warlock addon discussion. If there is sufficient interest/contribution and the forum moderator doesn't mind I'll maintain an OP here with "recommended" and useful addons and links to specific UI screen shots to be found in this thread. If I need to maintain a list of complete warlock UIs I will as well.

It allows you to switch on upto 4 different sound triggers for when Nightfall procs - one at the start, one halfway through the proc, one 3 seconds from the end, and one at the end if it drops off before you use it.
Gives a text warning and/or sound upon a very customizable variety of events. (Essentially, anything you can think of that would show up on the combat log). In one addon, it can completely replace the what DS Warning does, as well as what Power Auras does (although, it doesn't execute it as graphically) or Buff Watcher, etcetc.

As a warlock, I cannot live without Dominos_BuffTimes and OmniCC. Both allow me to use a combination of a simple counter and aura on top of the button. IMO, it's much more efficient than cluttering your UI even more with extra bars, such as DoTimer. It also allows you to be extremely precise since you're counting down to the very last second in real time. You can see an example of the aura and timer in the screenshot above with Fel Armor.

I took screenshots of the addons that I use most often and heavily rely on. I have edited each screenshot to indicate which addons are which so click on them to view highlights of each! Read below for a full list of my addons (: Well, not full because in the middle of listing everything, I've decided to take out the obvious addons such as QuestHelper. A few that may seem obvious such as Auctioneer or Atlas Loot, I would like to point out neat features that I really like ;D
ACP: Adds an "Addon" button to your main menu for easy access in order to enable or disable addons with a click of the "Reload UI" button. Especially handy in between trash pulls in a raid and you just realized you disabled your damage meters.
AtlasLoot Enhanced: Lists the loot dropped from bosses or acquired from events. One thing I really like is that I can make a wishlist of items that I would like to roll for so I can quickly double check if I wanted to roll for an item that has dropped.
Auctioneer Advanced: Scans the Auction House and stores its data viewable on item tooltips. I mainly use this add on to sort auction listings by PCT.
Butsu: Replaces and repositions the loot frame with a sleeker and smaller frame.
Chatter: Highlights names by class colours, time stamps messages, indicate character levels, etc. etc. Similar to Prat, but smaller in size. I really really want to list this mod because it is way better than Prat (;
DrainSoulTimer: Plays a sound and warns you either in chat, SCT, raid warning, to start drain soul. Plays a short "tic" sound whenever drain soul deals damage.
Enchantrix: Numerous enchanting options. I mainly use it to check out how much mats are worth for an enchant and how much I should charge for an enchant in my profession window. I listed this one because I thought many warlocks would be enchanters.
Enen: Bypasses the 60 second limit on the chat flashing tabs whenever you receive a whisper. Especially during a raid, you will not be immediately switching tabs to view your whispers.
EngBags: Wonderful bag mod that combines your bags into one, auto sorts your items, has a search function, and allows you to view your bank from anywhere. I have tried every single appealing bag mod out there and this one is definitely the best. I have no idea why not many people use it ):
Examiner: An inspection mod that shows you the details of a character's gear, stats, talents, and arena teams. You can quickly compare your spellpower to another warlock's. The best inspection mod out there!
FailBot: Really cute mod that announces in a chosen channel every failure occurred in the raid. Examples: Getting hit by Sartharion's Lava Waves, Kel'Thuzad's Shadow Fissures, Deconstructor's Gravity Bombs, General Vezax's Shadow Crashes.
Fane: Really pretty and minimalist chat tabs. They actually match a warlock very well due to its purplish/pink text colour!
ForteXorcist: A package of addons very useful to warlocks. I also believe they have addons for other classes as well but I have not tried them out. Anyways, includes a spell timer, cooldown timer, soul shard count, soulstone tracker, summoning assistant, healthstone tracker, and CC announcements.
InlineAurap: Displays a glowing border around spells on the action bar that are already casted on either the player or on the target. Example: Haunt will have a glowing red border if Haunt is already casted on the target or Fel Armor will have a glowing green border if Fel Armor is already casted on the player. Very useful for a quick check if your Life Tap glyph buff is up.
OmniCC: Displays a countdown on a spell on the action bar indicating its cooldown.
Outfitter: By far the best gear organizer. You can save sets of gear you can quickly switch in and out of. My sets are: Spell Hit 10%, Spell Hit 13%, Durability (it takes off all my gear), Noblegarden (bunny ears and such!), Winter Veil (santa hat and boots!), Resilience (PvP gear!), Tank (Sartharion and 3 drakes). It will also automatically switch your gear when you switch between your duel specs.
ScrollingCombatText: The best combat text mod I have used. Very useful for warlocks who are struggling to keep up the Life Tap glyph buff. It will show "[Life Tap]" whenever you life tap and "-[Life Tap]" whenever the buff fades.
Tactics: Prints boss descriptions in a chosen chat channel. I do not believe it is updated for Ulduar but you can easily add more bosses to the file yourself. I personally edited every boss according to our guild's executions. Very useful when you have a new recruit raiding with you the first time.
tekJunkSeller: Automatically sells grey items in your bags when you visit a vendor.
teksLoot: Replaces the need/greed rolling frames with a more minimalist and prettier frames.
Tenmetsu: Makes chat tabs flash whenever a message is sent, not only whispers. I personally have five tabs: chat, channel, private, group, and log. I really liked how Enen mod I mentioned above highlights the tab when I receive whispers. This mod makes it so it works with any type of message you want! Again, very useful during a raid when you are viewing your group/raid tab and someone says something in guild/officer.
Xperl: Definitely the best unit frames out there. I have tried all the popular ones but they were never satisfying. Although it may seem very ugly when you first use it, you can definitely take a few minutes to customize it to have the similar and popular look as Pitbull and others. Check out my screenshots to see (: One thing I love about this addon is that it includes a cast bar with latency indicator and also the useful distance indicator that fades someone's frame if they are far away (take advantage of it to quickly send out summons! or if you know you are suppose to be running away from someone with a harmful debuff, you can check if you are far enough from them in the first place so you do not have to spend the looking for the person and running the opposite direction!).

I use it mainly for mounting and very nice audio warnings ("Shadowtrance" i.e.) oRA3 (Version ---): Raid Assist Addon (MT Targets i.e.) in development, I recommend using oRA2 for the moment Pitbull Unit Frames 4.0 (Version ---): Unit Frames Addon in development, nevertheless the most recent version is working pretty nice. Highly adjustable addon - take your time for setup! Prat 3.0 (Version r302): Chat Addon (donno what to say - use it ) Titanpanel (Version 4.2.1.30100): Puts small informational bars on top and/or bottom of your screen. Easier to use than FuBar imo ... Other Addons like SexyMap, TipTac, Auctioneer Suite, Grid, Recount or Scrolling Combat Text (Damage), i.e., are also very good addons but maybe more nice-to-have than must-have. Depends on personal preference.

Maybe I misunderstood the OP, but perhaps we shouldn't clutter the thread with random lists of DKP/mail addons and stick to class/role related mods?
Anyway, I just noticed that Dr. Damage has a new version out after a long period of no updates and being broken since 3.1.
What it does: "DrDamage displays the calculated, average damage or healing of abilities with talents, gear and buffs included on your actionbar buttons. The addon will also add various information to the default tooltips in your spellbook and on the actionbar."
What is the utility of DrDamage? To know how much, on average, spell will cause damage? What for? We have the simcraft to calculate optimal talents build, and i trust to him more.

I'll be pulling the ones that aid warlock performance, so I'll subjectively filter as best I can. I'll link to the relevant posts where UI shots are and describe things as well as I can.
Most of the addons I use are already listed and my UI looks quite similar to some of the above as well, so I will just comment on one thing that I do different.
I used to be a great fan of Necrosis until it got some issues a long while back. Since then I have been using Lunar Sphere, which is not warlock-only but in my opinion a lot more flexible. Another advantage is that you can also use it to save space for 'general' spells and uses, for example: it can automatically select your best food and water so you don't need multiple buttons for those. It would be a too long list to include all the possiblities, just check the site if you are interested. There is a warlockmodule included to get you started.
I have 3 buttons setup to create a hs/ss/spellstone on left-click and use the item on right-click and one to summon the last-summoned pet on left-click and open the pet-menu with a right-click (and some more buttons for food, pots, mounts, etc.)
Warlock-specific addon is ShardAce WotLK -- its in the bottom-right, just above the dark panel. It handles summons, stones of all types, and pets (think Necrosis without the bloat).
I found and installed ButtonTimers a few days before this post. I used it to raid once, but to me it seemed like I was having conflicts with the other lock in my raid. For instance, she would cast Curse of Agony and my timer would reset. Have you or anyone else had this problem with this add on? I really liked the way this addon was setup. I would like to use it, but obviously with the conflicts mentioned here I had to switch. As a result I used a combination of quarts and DoT timers.
This screenshot represents my current UI. I’m not going to post about all the BS addons I use ill just mention the ones that I feel are the most useful to playing a warlock.
DoTimer – basic timer bar mod for dots/cooldowns among other things. Many use Forte warlock which basically does the same thing. DoTimer : WoWInterface Downloads : Warlock
Class Timer – Another timer bar mod. It is nice as you can track anything you want with it provided you know the buff name. I use it to track buffs like the LT Glyph and IDS. It can be used to track dots as well. ClassTimer - Addons - Curse
Everything else is pretty standard. Feel free to PM me questions if you have them.
Debuff Filter is my absolute favorite dot timer. I thought I would post it and let some of you warlocks know there is another dot timer you can try out if you are looking for another solution to managing your dots.
However, since 3.1 came out, Debuff Filter has not worked properly. With that said, I talked to the creator of Debuff Filter (which is available on wowinterface.com) and he says a new version is in the works!

The addons I consider essential for raiding as a warlock are : Pol Monitor(the 4 icons in the lower part of my hud). It allows me to setup icons for the spells I choose on which it will show me if I`m in range, I have mana, if I already have it on a mob it will show me the duration. The icons are click through and can hide when not in combat. Cooling (the icon in the upper part of my hud) is a pretty basic cooldown monitor that allows me to setup the icons however I want and restrict the category of spells / items / etc that will monitor. This is a very old addon and I couldn`t find any link for it.
I switched from Quartz to AzCastBar and I`m missing the show ticks option from it. I installed ForteExorcist just for that but having 2 casting bars is pretty annoying.
If anyone knows another casting bar replacement with tick support please let me know. If you have any other questions about my UI send me a pm.

1) is Forte Xorcist's Dot timer, Which Shows the next upcoming tick of dots and is filtered to my dots only (unlike the other defaults like powerups and beneficial debuffs)
2) is Forte Xorcist's cooldown bar, which as you may have noticed shows cooldowns in a linear fashion, and though it can be used to show buffs etc, I find that too clustered. The secondary splash Icon pops in the center of my screen.
3) Is a custom buff bar created by Satrina Buff Frames which also accounts for all my other buffs and debuff frames. This particular one is for Lifetap buff. It is created very simply by Creating a new buff bar, going to spells-> life tap-> show this buff. And then in settings makeing sure the bar is set to show only white list, then using the visual settings to convert it simply into a single slim bar.
4)_This is a second custom buff frame created by Satrina Buff Frames to show only useful buffs/debuffs. It's nice to keep a heads up idea of what procs buffs you have, in this case you can see lightweave, Illustration, and Decimation. It's very handy when tied with power auras to keep track of all your immediate use buffs.
5) These are more of the warlock tools from Forte Xorcist When not in combat you would see a summon monitor and you can quickly pop up from that list to see whose Battle res and ahnks are ready as well.
One thing that can't be seen is I use the following dog tag to monitor warlock friendly debuffs, Including 13% increased dmg, +3% hit and 5% crit debuffs. Note Dog tags work with Pitbull 3.0 and may or may not work with other addons in the same way.

The character linked in your profile appears to be below level 10. This may account for your poor Patchwerk DPS.

ButtonTimers is IMO the best timer addon for people new to affliction. The timer bars are clickable, so if you see one running out, you can simply click it to reapply.
You MUST have the most recent version of this mod, Kaylum. After making sure of that, you need to make sure that in the options the "Show Spells Cast by Other Players" option isn't checked.
One of my favourites is a little gem called qNightLight. It allows you to switch on upto 4 different sound triggers for when Nightfall procs - one at the start, one halfway through the proc, one 3 seconds from the end, and one at the end if it drops off before you use it.
I only use the first and the "3 seconds to go" option, and I changed the soundfile for the first to use the "ShadowTrance!" from Necrosis. So, without Necrosis' bloat, I have the groovy ShadowTrance vocal when Nightfall procs, and a little beep that plays 7 seconds later if I haven't use it yet.
Inline Aura is a new favorite of mine. It highlights the borders of your spells, if the effect is present on your target (and can watch focus too). In addition, it can show the current duration of your spells in their respective buttons. It's like having a dot timer on every button, and saves a lot of screen-real-estate.
The best part is that it's customizable. You can set it to show Decimate durations on your Soul Fire button. Or you can have it watch Nightfall instead of Shadow Mastery on your Shadow Bolt button. Any buff or debuff on you, your target, or your focus can be tracked and assigned to any spell.
In addition, if you use OmniCC, it will displace the InlineAura timers under the OmniCC timers, so you have both in the same button. I like this particularly for Haunt, as it shows both the cooldown and duration the same basic space.
This addon was mentioned before, but I thought it deserved better coverage.
MeesterHat briefly mentioned Power Auras Classic. With so many different talents, items, and spells having various effects, I found that important procs were getting lost in the myriad of other buffs in my main buff frame. While many of them are superficial and aren't necessarily important to what I'm doing (such as Dying Curse procs and perhaps the IDS stack), other procs such as the Life Tap glyph, Decimate, and Backdraft are ones i like to keep a closer eye on. The mod is relatively easy to setup so I won't go into detail, but you can easily add proc notifications anywhere on the screen. As Supplicium said, Satrina Buff Frames can achieve a similar effect. Six of one, half dozen of the other, I presume - though PAC will require more precise positioning on a proc-specific basis.
Interesting topic. It's always nice to see which warlock specific addons there are.
I've made a compilation for my warlock that I think provides a good overview in raids, doesn't use too much screenspace and is centered around the most important things to know during raids for a warlock.
The warlock specific addons used in the compilation is:
ForteXorcist - Dot timer which also shows Misery, Ebon Plaguebringer and Earth & Moon on your target. I also make use of the Cooldowntimer in the package.
Another Warlock specific addon I use is Demonic Knowledge which shows the spelldamage gained from that talent and updates when you recieve buffs.
To make sure I don't miss decimate procs, (which has been known to happen) I implemented Eventalert in my UI - It's not that configurable but you can set it to show procs such as Molten Core, Decimate, Nightfall, Backlash and Backdraft.
I also use Pitbull unitframes and Satrina Buff Frames. In the compilation I have added filtering for my demons so that it shows player casted buffs such as GotW, Fortitude, Intellect, Blessings and self buffs like Demonic Frenzy. I've also added Fel Armor, Demon Armor and spellstone/firestone, elixir/flask and well fed buff to another buff bar to make sure I don't forget to buff myself.
I use Parrot for my scrolling combat text which is configured to show incoming damage, damage done by my pet and incoming heals on me and my pet. I use the ingame damage done by me and my pet since I like to see big yellow numbers explode on my screen.

While this addon requires a bit of configuration, it also lets you create amazingly configurable buff/debuff monitors.
This screenshot shows my on-screen setup in my 3.0.8 affliction spec (now that Immo and UI don't stack and that SL is gone, I've changed it, I just don't have a more recent SS):
A few notes:
- I arrange the buttons to map in the same relative positions as the buttons on my n52TE which is why they are in two rows. You can reorder them however you want.
- Red means the spell is MISSING from the target
- The number is a countdown timer for how much time is left on it
- You can configure it for target, player, pet, etc. I have another one I setup for myself that flashes a big icon on my screen when Nightfall (affliction spec) or Decimate (destro) proc for me.
- I do the same thing to know when my LT buff drops.
This addon all by itself boosted my DOT uptime tremendously.
Like Auracle, it isn't a Warlock specific addon, but it has some nice warlock specific features.
"Cork provides, at it's heart, one-click buff casting. Some non-buff reminders are included, however, and more are planned."
For warlocks, in addition to the basic stuff like "Well Fed", repair, etc., it also has reminders and one-click resolution to the following:
- Summon pet
- Fel Armor
- Soul Link

When that window pops up, I simple click the Cork button (on my normal action bars) and it starts working the list top to bottom on each click. Fully configurable, so if you don't like a buff or want to specify a specific pet, you can do that.
Helps you not forget those things you need to do before engaging the boss.
ButtonTimers is probably the best addon I've found for my warlock. I used to use Inline Auras (which is also great) but I prefer a more visual display.
