If you get more than one PS effect within 8 seconds, the ticks will simply stack and do increased damage. There is no reason to ever delay anything. It has been working like that since wrath.

Pets should be switched depending on the fight. Worm is actually a poor choice when you have to aoe a bunch of MOVING adds, for example on H Maloriak, while Chimaera handles that job very well.

Around 90 seconds ICD. The bad thing is: the proc is completely random. Get unlucky and you're stuck with sub-par stat. I'd rather take the 258 version of Death's Choice, at least I know what I get :)
